The Department of Pharmacy Practice at ACPHS invites
applications from qualified candidates to join the College for a
non-tenure track appointment. Consideration of rank will be
commensurate with experience.
The faculty member will develop a patient-centered, team based,
clinical pharmacy practice site. This position will be based in
ambulatory medicine with the Albany Medical Center’s Internal
Medicine Group (IMG), a faculty practice affiliate of the Albany
Medical College. There are 10 total faculty physicians and two
advance practice practitioners (physician assistants, we are in the
process of hiring another). Two of the physicians are
board-certified Geriatricians and one has a home care practice that
serves 11 counties. One of our physicians is board-certified in
Obesity Medicine. The practice serves as the outpatient training
site for half of the Internal Medicine Categorical residents (23
residents, PGY1-3), Geriatrics Fellows, Medical and Physician
Assistant students. Residents engage in a weekly academic half day
(“team based learning” ) onsite and this has been a great
opportunity to engage interprofessional education. Medical students
in all four years of training will rotate at IMG starting
in 2023. IMG is an NCQA certified medical home
and participates in Primary Care First. The practice also has a
psychologist and cardiologist who are scheduled patient visits
weekly at the site. IMG is a mature but growing practice,
last year we saw 15614 follow ups and 1883 new patients. The
practice accepts a wide range of insurance policies including
Fidelis, NY State managed care. As part of medical home, the
practice has an active case management program based on the Mattice
risk method. There is a wide payor mix, including NY state Medicaid
and multiple commercial insurers. Prior authorizations at the
practice are managed by nursing and administrative support
staff.
Knowledge, Skills and Abilities:
• Knowledge of teaching and learning pedagogy and student success
initiatives.
• Knowledge of academic programming related to pharmacy
education.
• Ability to organize work effectively, conceptualize and
prioritize objectives, and exercise independent judgment.
• Ability to communicate and present information effectively to
internal and external audiences.
Pharm.D. degree from an accredited College of Pharmacy
• PGY1 residency experience,
• Eligibility for licensure in NYS.
PGY2 in area of specialty clinical practice preferred
