The Transition Program Specialist, under the supervision of the Student Success Manager, will assist in implementing the Early School Leavers Transition ( ESLT ) grant program objectives and be responsible for program outcomes. The position will help in recruitment of program participants, identification of student’s career goals, completion of the students’ High School Equivalency preparation classes, attainment of the students’ HSE certificate, and transition from Adult Education to college or employment. In addition, the Transition Program Specialist will be responsible for proactively supporting a caseload of out-of-school youths age 16-24 years old participating in the ESLT program. The Transition Program Specialist will work with Adult Education faculty and staff for student’s enrollment, testing, retention, and support services. Also, the position is responsible for coordinating and partnering with various academic and Student Development departments to ensure smooth transition to academic career & technical education ICAPS programs and student support programs. The position will also work closely with the JIC Community Development Corporation and the Job Center of Lake County in recruiting for program student participants and providing the students with community, job readiness, and job search services.

Required Qualification
  1. Bachelor’s degree.
  2. A minimum of two year’s experience working with secondary or post-secondary students, or adult learners.
  3. Data tracking and analytical skills.
  4. Excellent verbal and written communication skills.
  5. Ability to work with the diverse academic, socio-economic, cultural, and ethnic backgrounds of students, faculty, and staff.
  6. Proven ability to work effectively and constructively with persons of diverse cultures, language groups, and abilities; demonstrate sensitivity to and ability to work with the diverse academic, socioeconomic, cultural and ethnic backgrounds of community college students, faculty, and staff, including those with disabilities; establish and maintain effective working relationships with those contacted in the course of work.
Desired Qualifications

  2. Strong coordination skills.
  3. Bilingual in English and Spanish.
  4. Experience with case management software or data enterprise systems.

Department Operations & Procedures: 50%
  1. Coordinates with the Adult Education Enrollment Center for intake, testing, registration and New Student Orientation of students participating in the grant.
  2. Coordinates with Academic and Data Operations for ABE / ASE class scheduling and student records input into DAISI and PeopleSoft systems.
  3. Works with the Adult Education Academic Operations Manager and the other academic divisions’ Academic Operations Manager to ensure that the CTE and IET classes are set up for the term appropriately.
  4. Provides case management for the students participating in the grant by coordinating with their Adult Education and college credit faculty about the students’ progress.
  5. Assists in the registration of adult education students into ICAPS classes, developmental classes, and GECC classes. Monitors their progress in these programs towards completion.
  6. Conducts evaluations of grant projects and career student success initiatives to interpret the level of effectiveness against established criteria. Share the evaluation with the external partners of the grant.
  7. Collaborates with internal and external stakeholders to interpret grant, propose grant activities, and implement grant initiatives
  8. Prepares quarterly and end of year reports to submit to ICCB .
  9. Assists the Student Success Manager in drafting the grant application by providing activities, data, actual and expected outcomes, and program improvement plan as needed.


Student and Customer Service: 45%
  1. Works with the internal and external partners of the grant to recruit youth ages 16-24 years to participate in a cohort of 55 students.
  2. Coordinates with the JIC Community Development, Youth Conservation Corps, and with the Job Center of Lake County youth programs for services that can be provided to the students recruited from these organizations. Provide data and information to these partners about the progress and outcomes of these students.
  3. Provides academic coaching to students regularly in the assigned caseload, documents all student interactions in the CRM, refers students to tutoring services and other support programs that will contribute to a student’s academic or personal success.
  4. Assists the students in determining their career goals by assisting them to register in Bridge to Careers classes, career exploration activities, and participation in workforce preparation events.
  5. Provides transition services to college to students by coordinating with the On-boarding Navigators and Field of Interest Navigators to set up appointments, the Financial Aid office to complete financial aid and scholarship applications, and the various college departments to provide New ICAPS Student Orientation and other orientations programs
  6. Refers and assists students for job services to the Adult Education Student Success Coordinator responsible for job service services and the Job Center of Lake County. Refers and assists students to the Career and Job Placement Services for apprenticeship and internship programs

Other: 5%
  1. Participates in relevant committee meetings.
  2. Attends professional development trainings.
  3. Performs other duties as assigned.
