May identifies physical, psychosocial, spiritual and social needs and coordinates the development and implementation of a diabetes focused post discharge plan of care to meet those needs effectively, efficiently and at the appropriate level of care required.

  • Work with leadership to develop, Integrate / implement Continuous Glucose Monitoring (CGM) pre and post-discharge workflow.
  • The position is goal-oriented with a high level of accountability correlated to the reduction of readmissions, continuum of care, quality of care, and patient satisfaction.
  • Coordinates care for a group of primary complex patients to achieve clinical and organizational outcomes.
  • Provides evidence-based care based on nursing process, policies, procedures, and protocols of the Johns Hopkins Hospital.
  • Provides direction to/management of other team members to promote quality, safety, and excellence in care.
  • Actively participates in Comprehensive Unit Based Safety Program (CUSP) initiatives.
  • Works collaboratively with a multidisciplinary health care team to optimize team performance and patient outcomes.
  • Triages phone calls and electronic advice requests from patients and determines need for medical assessment.
  • Identifies care continuity needs of patients; generates and pends orders to primary providers for signature.
  • Provides patient education for self-care chronic disease management (in person, by phone, or via electronic record).
  • Responsible for other clinical duties as assigned.

Minimum Qualifications (Mandatory):

    Must be a registered nurse, licensed in the State of Maryland. Bachelor’s Degree in nursing or related discipline preferred. Must maintain active licensure throughout employment.

  • Expected to learn the formal and informal departmental goals, standards, policies and procedures which may include some familiarity of other departments within the school/division. Is sensitive to the interrelationship of both people and functions within the department.
  • Knowledge of current health care environment.

  • Highly effective verbal communication and interpersonal skills to establish working relationships that foster optimal quality patient care.

  • Ability to interact successfully with the public. Ability to perform effectively despite sudden deadlines and changing priorities. Ability to maintain personal composure in high stress situations.

  • Ability to demonstrate self-direction and initiation.
  • Ability to work independently and as a part of a team.

Physical Requirements:

    Possible exposure to communicable diseases, hazardous materials, radiation, and pharmacological agents. Ability to move about or sit for an extended period of time. Work requires lifting of 35lbs without assistance, unless special accommodations are in place. When lifting weight above 35lbs assistance or assistive lifting devices should be used. Reaching by extending hand(s) or arm(s) in any direction. Finger dexterity required to manipulate objects with fingers rather than with whole hand(s) or arm(s), for example, using a keyboard. Communication skills using the spoken word. Ability to see within normal parameters. Ability to hear within normal range.
