We are currently seeking to fill our Assistant/Associate Professor- Ambulatory Care, Clinical Pharmacy Specialist position at the Philadelphia campus with a highly qualified and passionate individual.

Philadelphia College of Osteopathic Medicine School of Pharmacy (PCOM-SOP) is dedicated to developing leaders in pharmacy through an engaging and immersive learning environment supported by academic departments holding a commitment to innovative research, teaching, and professional and community service.

The PCOM-SOP invites applications from highly motivated candidates for a full-time Assistant/Associate Professor, Clinical Pharmacy Specialist faculty position in the Department of Pharmacy Practice with a specialized practice area in Ambulatory Care Family Medicine. This is a full-time faculty position located in Philadelphia, Pennsylvania at the PCOM Healthcare Centers. Position reports to the Chair of the Dept. of Pharmacy Practice.

Description

The primary role of this position will be focused on the development and implementation of ambulatory care pharmacy services as part of an interprofessional healthcare team at PCOM Healthcare Centers in Philadelphia, PA. The Clinical Pharmacy Specialist (CPS) will be responsible for developing medication adherence strategies, utilization management, immunizations and chronic disease state management practices at one location. The primary objective of the position is to develop relationships with patients and the patient’s caregivers to improve health outcomes and quality metrics through primary care referral appointments, curbside interventions, team-based care, fee-for-service activities, and the creation and implementation of population health initiatives with the potential for merit-based reimbursements. The CPS will identify opportunities for practice improvement through value creation, cost control and utilization performance, and the development and implementation of strategic plans in collaboration with the primary care team. This position will also serve as a resource for healthcare providers and staff, and precepting PCOM experiential PharmD students. The position may also include teaching PCOM didactic PharmD students online, in a limited capacity, based on the demands of the institution.

Key Responsibilities

Demonstrates and maintains expertise in clinical pharmacy practice including appropriate drug therapy for patients and prevention/resolution of drug related problems

Optimizes drug therapy according to a collaborative practice agreement, where feasible, to include designing, implementing, monitoring individualized pharmacotherapy and documenting pharmaceutical care activities and outcomes.

Serves as resource to providers, care managers, and other healthcare professionals on complex problems, medication reviews and to assist in accurate completion of medication reconciliation activities.

Analyzes patient data to make decisions at the individual patient and/or population-based level, to identify target populations, conduct outreach to schedule appointments for pharmacy services, assess and resolve medication-related problems and to make patient-centered, medication and non-medication, health improvement, wellness, and disease prevention recommendations.

Designs processes and initiatives that support quality measures to improve population health outcomes of medication therapy, prevent inpatient admissions or readmissions, increase patient satisfaction, decrease unnecessary utilization costs, and to help institute PCOM Healthcare Centers as a medical home for patients.

Ensures continuity of care and access with all medications and refers patients to patient assistance programs, or other resources to navigate medication access issues, as needed.

Develops and fosters positive collaboration with providers and clinic staff.

Provides educational in-services on medication-related topics to patients, providers, staff, care managers, and other healthcare professionals.

Participates in developing guidelines, algorithms, and standard protocols to improve the care and safety of patients and monitor the effectiveness and quality of clinical pharmacists’ services and patient outcomes.

Develops and promotes advanced clinical pharmacy practice and presents/publishes research findings with regional and national associations.

Precepts and mentor students from the PCOM SOP.

Assists the organization in achieving compliance with accreditation, legal, regulatory, and safety requirements related to the use of medications, point-of-care tests, and immunizations.

Minimum Qualifications

Graduate of an accredited school or college of pharmacy.

Completion of an ASHP-accredited PGY-1 Residency or Fellowship, or equivalent experience in the specialty area.

Active Commonwealth of Pennsylvania pharmacist license or eligibility for licensure within 60 days of hire.

Desired Qualifications

Two years of clinical specialist experience setting up an ambulatory care practice, or other entrepreneurial experience.

Completion of an ASHP-accredited PGY-2 in ambulatory care or population health is preferred

Board certification (BCPS, BCACP, BCGP) is preferred
