Health Services Professional
Performs assessment, treatment planning, intervention, monitoring,
evaluation, and documentation on identified patient populations.
Assesses and develops a care plan in collaboration with the heath
care team. Effectively manages patients' medical, environmental,
and psychosocial needs to ensure their needs are being met.
-Participates with and engages patients in the assessment,
planning, monitoring and evaluation of their care needs.
-Documents patient assessments, care plans and other relevant
information in the electronic health record (EHR).
-Obtains and coordinates existing resources to meet patient
-Operates under nursing protocols
-Manages allergy injection reactions
-Assists with employee/ student bloodborne pathogen
-Coordinates and operationalizes outreach events for vaccination
and TB testing.
-Participates in the education of peers and learners.

Associate or Bachelor's degree in nursing
Minimum of one year of nursing experience
Excellent communication skills- verbal and written
Ability to multi-task and prioritize tasks
Ambulatory nursing experience.
Experience administering immunizations, ordering titers and
administering TST skin tests
Experience working with allergy patients who are receiving
Experience working under nursing protocols
RN - Registered Nurse - State Licensure And/Or Compact State
Active Wisconsin RN license, current BLS certification
BCLS - Basic Life Support
