Job Summary:

Performs assessment, treatment planning, intervention, monitoring, evaluation, and documentation on identified patient populations. Assesses and develops a care plan in collaboration with the heath care team. Effectively manages patients' medical, environmental, and psychosocial needs to ensure their needs are being met.
Principal Duties:
-Participates with and engages patients in the assessment, planning, monitoring and evaluation of their care needs.
-Documents patient assessments, care plans and other relevant information in the electronic health record (EHR).
-Obtains and coordinates existing resources to meet patient needs.
-Operates under nursing protocols
-Manages allergy injection reactions
-Assists with employee/ student bloodborne pathogen exposures.
-Coordinates and operationalizes outreach events for vaccination and TB testing.
-Participates in the education of peers and learners.

Education:

Required
Associate's Degree
Associate or Bachelor's degree in nursing

Qualifications:

Minimum of one year of nursing experience
Excellent communication skills- verbal and written
Ability to multi-task and prioritize tasks

Preferred experience:
Ambulatory nursing experience.
Experience administering immunizations, ordering titers and administering TST skin tests
Experience working with allergy patients who are receiving injections
Experience working under nursing protocols

License/Certification:

Required
RN - Registered Nurse - State Licensure And/Or Compact State Licensure
Active Wisconsin RN license, current BLS certification

Required
BCLS - Basic Life Support
