Summary
To provide program administration for Bridging the Gap, a multi-disciplinary, hospital-based violence prevention and intervention program, dedicated to reducing injury, re-injury and mortality rates among youth and adults by providing intentionally injured patients with support during their hospital stay and beyond. This position will oversee the Bridging the Gap program by providing program administration and development for all of Bridging the Gap to include case management teams, peer support teams and prevention programming. This position will also provide training and technical assistance to hospitals at the local, state and national level; including the development of training materials.

Typical Duties
  • Assist case management teams by providing oversight of the program managers
  • Strengthen processes for patient referral pathways
  • Establish communication networks between providers and Bridging the Gap
  • Expand and strengthen internal partnerships as it relates to hospital-wide integration
  • Develop policy, procedures and protocol to adapt for a growing program for all facets of BTG
  • Establish new partnerships to close service gaps and improve patient outcomes
  • Act in a leadership capacity for initiatives internal to VCUHS and at the local, state and national level
  • Oversee program expansion into the Tri-Cities region
  • Act as content expert for presentations and trainings at the local, state and national level
  • Expand prevention services
  • Provides supervision in coordination with other Directors/Managers for training and development specific to Dating Violence intervention and prevention services
  • Provide content expertise to participating hospitals
  • Assist sites with program development
  • Provide as needed assistance to sites
  • Oversee day-to-day training and technical assistance duties as they relate to community violence
  • Supervision of 9+ staff members



Required Education and Experience
Master’s degree in social work or related discipline. Ability to understand and work with those at-risk for violence and their families. Experience providing case management services. Knowledge of child/adolescent service system. Knowledge of positive youth development pathways. Ability to provide assessment, counseling, education/support to at-risk individuals and families. Ability to work collaboratively with families and community agencies. Excellent interpersonal and organizational skills.
Must agree to CPS and criminal background check. Dependable transportation required.
Ability to work in a diverse environment required.

Preferred Education and Experience
Program development experience and hospital and community experience preferred. Prior supervisory experience preferred
