Job Description: To case manage the delivery of quality services through the application of case management/care management principles in a community based model for children with special health care needs (CSHCN) which is family­ centered, culturally competent, and coordinated within a comprehensive integrated system of care.

Required Qualifications: Assessment Skills: Able to assess and navigate complex system issues to access community resources and interventions for achievement of optimal goals for families and their children with special health care needs (CSHCN). Utilize Case Management/Care Management skills, experience, and knowledge of complex conditions of CSHCN, their families and the systems issues that they encounter, is able to quickly identify and prioritize problems and appropriate interventions to quickly decrease their acuity level. Verbal Communication/Interpersonal Skills: Must effectively communicate electronically via the internet, telephone and face-to-face with internal and external customers. These contacts may involve difficult communications that require resourcefulness and understanding of community resources and services. Must be able to make presentations to small and large groups. Must be skilled in conflict resolution. Assertiveness: A willingness to be persistent to advocate for the needs of CSHCN within the VCUHS, community, and State. An ability to advocate for the needs of the client when faced with challenges. Able to demonstrate and mentor for clients, staff, and peers successful problem solving skills, negotiation skills and conflict management. Analytical ability/Problem solving Skills: An ability to relate and compare data from different sources to resolve complex human problems that require the use of considerable judgment and a professional level of knowledge. Able to demonstrate leadership in complex case management and the achievement of optimal outcomes. Leadership: Able to demonstrate leadership in complex case management and the achievement of improved outcomes for the population served. Able to mentor case management role and responsibilities as a change agent involving clients, systems, and the community. Flexibility: Must be able to adapt to changes in work roles and expectations in order to accomplish client/department goals. Written communication: Must be able to communicate information in writing that is clear and understandable to others. Able to utilize computer as a resource to achieve this skill. Experience with data collection and interpretation including the use of databases and spread sheets. Ability to work in a diverse environment required.

Preferred Qualifications : BSN for nurses; MSW for social workers. Certification in Case Management with two years of experience. Experience in Care Coordination and e.g. community social services, home health care, discharge planning and utilization management. Valid driver’s license & vehicle required. Bilingual skills are helpful.

Required License/Certifications: Graduate of an accredited nursing program. Current RN licensure in Virginia or eligible.
BSW from an accredited Professional Social Work program.
