Job Summary
The Caring Together Addictions Counselor, reporting to the Program Manager, provides direct counseling services and appropriate documentation for women enrolled in the Caring Together Program. Duties include intake evaluations, biopsychosocial evaluations, individual and group counseling, recovery planning, including treatment plans and ensuring that all appropriate documentation and clinical notes are completed in the patient charts within the required timeframe. Knowledge and acceptance of medication assisted treatment (MAT) is required.
This is a full-time 40 hours per week, benefit eligible position. The Caring Together Program is located in the East Falls section of Philadelphia at 4700 Wissahickon Avenue, Philadelphia PA 19144.
Essential Functions
- Provides direct client services including individual and group counseling for a caseload
- Maintains timely & accurate documentation in client files, including individual and group progress notes, treatment plans, discharge summaries, aftercare plans as well as related client correspondence
- Ensure all notes, releases of information, intake paperwork, including ASAM and treatment plans are filed in chart and up to date
- Ensures confidentiality with all cases
- Coordinates services with all of patients’ ancillary services.
- Meets productivity targets set in collaboration with Program Manager
- Ensures accurate billing
- Provides expert consultation to staff and community on women with addictive and psychiatric disorders
- Attends weekly staff and clinical supervision meetings to present issues and collaborate with the treatment team
- Coordinates services for mothers in the program to ensure appropriate childcare services are available
- Coordinates psychiatry and case management services for all patients on caseload
- Participate in treatment team meetings
- Performs other duties as assigned
- Bachelor’s Degree in Healthcare, HIV, mental health and/or substance abuse disorders
- Experience conducting individual and group therapy
- Knowledge and acceptance of medication assisted treatment (MAT)
- Must have access to an operating computer at home
- Master's Degree in human service field and/or Certified Addictions Counselor
- 2 years of full-time experience in the field of addiction counseling
