Bridging the Gap (BTG) is a voluntary program that focuses on Richmond area youth and their families. The goal of the program is to reduce the rate of re-injury, and mortality among youths and adults by providing intentionally injured patients at VCU Medical Center with support from a victim advocate, individualized goal planning, and access to resources in the community. Bridging the Gap works to break the cycle of violence. The BTG Manager will provide program coordination for the multi-disciplinary, hospital-based violence prevention and intervention program. Specific duties include:
  • Work with hospital staff to identify youth and adults admitted for violent injury and to engage youth, adults and families in a positive development program
  • Provide identified individuals with community based counseling, case management and other services post-discharge
  • Oversee the implementation of a crisis fund serving violently injured patient victims and their families
  • Analyze data patterns and trends
  • Provide accompaniment for victims community violence within the hospital, criminal justice system, and other systems as needed
  • Facilitate communication and networking amongst service providers to increase efficiency and effectiveness of direct services to victims
  • Work with program evaluators to monitor and assess performance in relation to stated program objectives
  • Manage administrative and fiscal operations for Bridging the Gap
  • Responsible for data, grants management, and other reporting
  • Participate in planning activities for Bridging the Gap to include long range and short range planning
  • Assists with the development and implementation of long and short term goals for the Bridging the Gap program and others
  • Supervise 5+ case managers and 4+ peer specialists


Required Qualifications

Master’s degree in human services or related discipline. Ability to understand and work with those at-risk for violence and their families. Experience providing case management services. Knowledge of child/adolescent service system. Knowledge of positive youth development pathways. Ability to provide assessment, counseling, education/support to at-risk individuals and families. Ability to work collaboratively with families and community agencies. Excellent interpersonal and organizational skills.

Must agree to CPS and criminal background check. Dependable transportation required.

Preferred Qualifications

Program development experience and hospital and community experience preferred.
