Bridging the Gap (BTG) is a voluntary program that focuses on Richmond area youth and their families. The goal of the program is to reduce the rate of re-injury, and mortality among youths and adults by providing intentionally injured patients at VCU Medical Center with support from a victim advocate, individualized goal planning, and access to resources in the community. Bridging the Gap works to break the cycle of violence. The BTG Tri-Cities Case Manager will provide intensive case management services for the program. Specific duties include:

Provide hospital and community based services to patients within the VCUHS who have experience or are at-risk for intentional violent injury through assessment, crisis intervention, and counseling. Work with hospital and community providers to participate in determining needs of patients. Provide identified individuals with case management services post-discharge. Analyze data patterns and trends. Oversee the implementation of a crisis fund serving participants enrolled in the program. Assist with coordination and facilitation of a youth leadership program. Assist with coordination and facilitation of youth and adult service providers in the hospital and community. Facilitate communication and networking amongst service providers to increase efficiency and effectiveness of direct services to victims. Work with program evaluators to monitor and assess performance in relation to stated program objectives. Responsible for data, grants management, and other reporting. Participate in planning activities for Bridging the Gap to include long range and short range planning. Assists with the development and implementation of long and short term goals for the Bridging the Gap program and others.

Required Qualifications

Ability to understand and work with underserved communities, at-risk youth and families in the Tri-Cities area. Knowledge of child/adolescent services system. Knowledge of positive youth development pathways. The ability to provide assessment, brief counseling, education/support to youth and adults. Ability to work collaboratively with families and community agencies. Successful experience working with young people in an organized programmatic setting. Demonstrated proactive problem solving, teamwork, innovative thinking, creativity, patience and flexibility. Demonstrated commitment and ability to support and mentor young people in a positive manner. Must agree to a CPS and criminal background check. Must have reliable transportation. Availability to work some evening and weekend hours.

Preferred Hiring Standards

Previous employment or volunteer work in human services, community agencies or related non-profit or social change organization a plus; experience with probation/parole or criminal justice system preferred. Demonstration of strong working relationships in the Tri-Cities area. Hospital and community experience preferred.

This is a restricted position with no set end date and continued employment is dependent upon project need, availability of funding, and performance.
