Johns Hopkins Bayview Department of Addiction Medicine is seeking a Community Health Worker (CHW) (Outreach Worker) who will work directly with patients served by the Comprehensive Care Practice and engage in diverse activities with the goal of supporting interprofessional, individualized, and dynamic plans of care for patients who are HIV+. The CHW will actively support patients in their path to needed services within the Johns Hopkins System and across the Ryan White Eligible Baltimore Metropolitan Area (EMA). This individual will work with a team of other CHWs to enhance navigation services within HIV/AIDS and substance use disorder treatment services. The target population for this position is to work with adults across all age groups.
Specific Duties & Responsibilities:
- Initiate, build and maintain relationships with patients in the clinic, offering support and guidance in medical and non-medical supportive services that are available in our program and in the community.
- Assist clients in navigating a complex medical system by building trust and a professional working relationship.
- Assess the patient’s strengths as well as needs to address individual and community barriers to care and intervene as appropriate.
- Provide support and encouragement and good self-care practices which include appropriate HIV care and substance use treatment.
- Help coordinate and potentially accompany participants to medical, substance use treatment, and social service appointments.
- Work collaboratively with healthcare providers and meet with hospitalized, homeless and community dwelling adults who are newly diagnosed or have become disengaged; providing support and encouragement about living healthy with HIV, including other diagnoses.
- Provide resource materials to patients and their families.
- Execute referrals for HIV services as requested and recommended by the interprofessional care team.
- Assist with administration of annual patient satisfaction surveys.
- Plan and conduct quarterly health promotion events for patients in support of risk reduction.
- Assist with tracking information and general record keeping
- Provide direct service to consumers, as well as those who have fallen out of care. Document all services provided in the Electronic Medical Record.
- Telephone contact with patients and community based agencies.
- Attend and participate in regularly scheduled staff meetings and conference calls.
- Work closely with other CHWs and Nurse Case Manager (NCM) to plan and conduct navigation activities.
- Go into the community or
to a patient home or to hospital room to locate out of care
patients to re-engage them into care following established policies
for outreach activities for identified
patients.

High School Education with
a minimum of two years’ experience.
Dependable, energetic,
self-starter.
Complete and pass JHU SOM
requirements.
Ability to travel locally
if needed.
- Completion of formal CHW training program preferable.
- Experience working with people with unstable housing, substance use issues, dual diagnoses, and HIV/AIDS preferred.
- Knowledge of current HIV treatment guidelines preferred.
- Proficiency with MS Word preferred.
- Excellent writing and interpersonal communication skills.
- Ability to communicate effectively with a wide variety of individuals from patients to administrative personnel, clinicians, and physicians.
- Ability to adhere to policy and procedures and maintain clear professional boundaries.
- Attention to detail and ability to chart and enter data accurately.
- Bilingual (Spanish) is preferred but not required.
- There is little likelihood
of exposure to communicable diseases, hazardous materials, or
pharmacological agents. Activities include but are not limited
to:
- Sitting in a normal seated position for extended periods of time
- Reaching by extending hand(s) or arm(s) in any direction
- Finger dexterity to manipulate objects with fingers rather than with whole hand or arm, for example, using a keyboard
