Job Summary

Under the supervision of the Director of Community Health Partnerships and Navigation, the Community Health Worker (CHW), under the PHA CARES: COVID-19 Action Research and Educational Services program will provide services primarily to residents in Philadelphia Housing Authority developments and Housing Choice Voucher (Section 8) residents. The CHWs will conduct COVID-19-related health education and strategies to prevent transmission of COVID-19, including explaining promoting social distancing, wearing of masks, using hand sanitizers, and the purpose of contact tracing. The CHWs may be asked to make phone contacts and conduct outreach using various virtual and technology strategies (including texting, emails, zooms) to enhance communications between ambulatory care, clinic patients, and the health care team, to improve participation in the plan of care and ensure positive health outcomes. Interaction between the CHW and the patient is designed to promote positive behavior change, such as protocol adherence, regular health monitoring, and the importance of addressing health issues.

Essential Functions

The Community Health Worker under the PHA CARES (COVID-19 Action Research and Educational Services) program will provide CHW services to residents in Philadelphia Housing Authority developments and Housing Choice Voucher (Section 8) residents. In the context of COVID-19, the CHWs will be responsible for the following functions: Communication

• Articulates thoughts and ideas effectively both verbally and in writing.

• Restates the opinions of others to clarify understanding.

• Presents information effectively in a variety of settings: one-on-one, small and large groups, with peers, direct reports, and superiors.

• Listens actively and is open to feedback. Provides fair and constructive feedback to others.

• Keeps team members and customers adequately informed.

Community Outreach

• Attends community meetings or health fairs to understand community issues or build relationships with community members and patients.

• Distributes flyers, brochures, or other informational and educational documents to inform members of a targeted community about the clinic.

• Conduct outreach and engagement to identify the residents to invite community conversations

• Attend meetings in the various housing developments to describe this project

• Build relationships with the leadership staff in the various housing developments to gain support and assistance

Supporting Patient Self-Care

• Makes regularly scheduled phone contact or home visits with assigned patients to monitor and assess their needs, and to make sure they comply with the required or recommended care plan.

• Identifies potential health risks, such as lack of food, heat, medications, and other services.

• Regularly meets and reports back to the Director of Community Health Partnerships and Navigation for intervention.

• Supports changes in wellness attitudes by encouraging patients to talk about their conditions, what they most want to change, what support they have to foster change, and what obstacles or difficulties must be removed or minimized to advance healthy behaviors.

• Interprets, translates, or provides cultural mediation related to health services or information for patients.

• Electronically maintains updated client records with plans, notes, appropriate forms, or related information.

Professional Development

• Participates and completes a minimum of 75 hours of core training for a Community Health Worker for over five weeks.

• Participates and completes a ten-hour COVID-19 Contact Tracing workshop.

• Participates and completes a Mental Health First Aid Training.

• Participates and completes a CPR and First Aid course.

• Participates in a 6-week on-the-job training component which includes at least 3 hours a week to meet as a group with other Community Health Workers.

• Attends professional development workshops offered by Temple's Human Resources or Computer Services Department.

• Completes a Professional Development Plan as a Temple employee.

• Meets weekly with a Nurse Navigator for supervision to improve quality of service.

Documentation:

• CHW will complete navigation and tracking forms, document all community encounters, maintain comprehensive electronic contact files, schedule appointments, and submit weekly reports to the Director of Community Health Partnerships and Navigation.

• Document and report incidents of child or elder abuse, neglect, or threats of harm to authorities, as required.

• Refers community members to needed health and allied health services.

• Assists families to apply for social services, including Medicaid, TANF, SNAP, housing, etc.

• Transports or accompanies patients to scheduled health appointments or referral sites as needed.

Evaluation and Transformation of the Health Care System - CHW

• Enters patient contacts into a spreadsheet

• Shares information about the health care needs of individuals in the community or target area with the Steering Committee.

• Provides feedback to health service providers regarding improving service accessibility or acceptability.

• Collects information and evaluation data from patients regarding satisfaction with services and overall conditions.

Accountability

• Takes responsibility for own actions.

• Strives to improve levels of individual, team, and organizational performance.

• Measures self against standard of excellence.

• Participates in performance improvement efforts.

• Demonstrates efficient and effective use of organizational resources as well as systems and services Other Duties and Responsibilities

• Complies with all Drexel University's policies and procedures, and all applicable accreditation standards, laws, and regulations, including those regarding patient confidentiality such as but not limited to, Health Insurance Portability and Accountability Act of 1996, P. L. 104-191 ('HIPAA') and the rules and regulations implemented hereunder.

• Performs all job responsibilities in alignment with the core values, mission, and vision of the organization.

• Performs other duties as required and completes all job functions as per departmental policies and procedures.

• Attends staff meetings and completes mandatory in-services and requirements and competency evaluations on time.

• Maintains current knowledge in present areas of responsibility (i.e., self-education, attends ongoing educational programs).

Required Qualifications
  • High School Diploma or GED
  • Must be a Philadelphia Housing Authority developments and Housing Choice Voucher (Section 8), residents
  • Completion of a minimum of 75 hours of Community Health Worker Core Training (Preferred)
  • General Experience in a medical or health-related field and knowledge of medical terminology (Preferred)
  • Comfortable communicating and connecting with community members of all backgrounds
  • Ability to assist with the navigation process and connecting people to services and follow-up
  • Good verbal and written communication skills
  • Passion for helping the community
  • Flexibility with scheduling including working on some weekends/evenings
  • Capacity to canvas neighborhoods on foot to engage in street outreach, walking, standing, and sitting for extended periods to engage community residents
  • Ability to accurately log and track daily activities/outreach
  • Ability to use computing devices (eg. Smartphones, tablets, laptops/desktops)
  • Knowledgeable about Microsoft word, Excel, Powerpoint,
  • Ability to analyze goals/outcomes
  • CHWs will be expected to travel throughout different communities in Philadelphia and to Philadelphia Housing Authority developments. Some of the tasks may be conducted remotely, depending on Drexel University's policies and procedures in response to COVID-19
