Job Description:

The Department of Neurology and the Normal Fixel Institute for Neurological Diseases are seeking candidates for a Licensed Clinical Social Worker position. The Licensed Clinical Social Worker independently provides casework services to patients and families as indicated to address the psychosocial aspects of care regarding adjustment to illness and treatment. Responsible for professionally applying social work methods and theory to the diagnosis, prevention, and treatment of psychosocial dysfunction, impairment, or disability, including mental, emotional, and behavioral disorders for families, individuals, groups, and organizations. This includes screening medical records, conducting interviews and assessments, setting treatment goals, and arrange for expediting and coordinating services. Works collaboratively with interdisciplinary staff internal and external to the organization. Participates in quality improvement and evaluation processes related to patient satisfaction and patient rights and serve as an information resource for the Fixel Institute for Neurological Diseases (FIND) in the Department of Neurology.

Duties to include:

Promotes and facilitates developing a specific treatment plan or strategy that reflects the therapeutic intervention that addresses the patient(s) need. Provides patient navigation, especially for patients moving among different health care levels (inpatient, outpatient, home-health, etc.). Provides education and serves as a leader for support programming; assistance with medications, transportation and advance directive; outreach and coordination with other community resources and agencies and community-level advocacy on behalf of patients and families. Conducts trainings for service providers and caregivers.

Responsible for the referrals seen and evaluated through the Deep Brain Stimulation (DBS) Fast Track Network program, Huntington’s Clinic, and others as assigned at the program for Movement Disorders and Neurorestoration. This includes identification, assessment and treatment of mental health conditions, such as depression and anxiety; and communicates clear and concise treatment goals and perform continued assessments to develop new treatment strategies when needed.

Responds expeditiously to emergency needs of patients and families by providing appropriate interventions necessary to stabilize, improve or resolve crisis situations. Demonstrates the ability to manage crisis situations effectively and to problem solve under pressure. Communicates to appropriate parties immediately upon discovery of potential problems, ensures “no surprises”; identifies priority or key issues in a complex problem or situation; uses existing practices and available resources to help solve problems; works to solve problems promptly, even with limited facts and resources. Substantiates social work services rendered by documenting in the electronic medical record (EMR) and maintaining accurate records and submitting other necessary documentation as needed.

Attends required meetings; participates in committees and focus groups; completes required reports in a timely manner and attendance of conferences/meetings as a representative of the FIND. Serves as back-up for case coordination in other divisions.

Expected Salary:

$58,000-$70,000

Minimum Requirements:
  • Master’s degree in an appropriate area of specialization and two years of experience; or a bachelor’s degree in an appropriate area of specialization and five years of appropriate experience
  • Valid Licensed Clinical Social Worker (LCSW) in the State of Florida
Preferred Qualifications:
  • Master's degree in Social Work (MSW) from an institution accredited by the Council on Social Work Education.
  • Experience in a healthcare setting preferred.
  • Professional empathy in dealing with families
  • Sense of professional identity while collaborating with co-workers
  • Awareness of community resources and programs for the disabled and elderly
  • Appropriate professional judgment, courtesy and tact
