The Department of Infectious Diseases is seeking a Senior Community Outreach Specialist (Sr. Peer Coach) who will support a grant to expand access to treatment of mental health and substance use disorders in community-based settings including the John G. Bartlett Specialty Clinic. They will facilitate outreach, linkage and retention in care for patients with infectious diseases, mental health, and/or substance use disorders. They will support a novel approach to Patient Centered Collaborative Care: maintain a caseload of patients; communicate patient status with the consulting addictions psychiatrist and with the patient’s primary care provider in Bartlett; support patients to identify and achieve realistic, patient-driven treatment goals. They will contribute to a productive, collaborative work environment that holds patients and coworkers, clinic staff and providers in positive regard. Reports to Program Manager.

Specific Duties & Responsibilities:

  • Demonstrates ongoing commitment to working as a member of a patient-centered Team based on a structured, team approach of collaborative care.
  • Manage a caseload of assigned patients.
  • Track assigned caseload of patients receiving care using a registry so that no patients fall through the cracks.
  • Use established measurements and treatment goals to support shared care plan agreed on by the Team.
  • Base Coaching on evidence-based treatments as communicated by the Team.
  • With Team direction, be responsible for patients and caseload reaching treatment targets.
  • Contribute to Team efforts in ongoing improvement in the quality of care patients receive for SUD and MHD through the grant.
  • Attend all scheduled Peer Coach training seminars, and pass exams on seminar content.
  • Demonstrate capacity to implement coaching and collaborative skills taught in trainings.
  • Administer standard evaluations to assess patient status at regular time points.
  • Meet with patients in caseload at scheduled time points.
  • Attend weekly and monthly case review meetings and communicate individual patient progress and goals to the treatment team.
  • Support patients in understanding their diagnosis.
  • Support patients in creating attainable goals related to their health and treatment of substance use, mental health, and infectious disease.
  • Support patients to adhere to treatment plans and recommended medical care.
  • Access patient adherence and treatment plan related information in the Electronic Medical Record with demonstrated ease (training provided).
  • Accurate and prompt documentation of interactions with program clients.
  • Some reporting and data entry needed via computer.
  • Driving to clients and community partners may be needed.
  • Maintain availability to assigned patients in caseload by phone (provided) during working hours.
  • Maintain healthy boundaries and self-care.

Special Knowledge, Skills & Abilities:

  • High School Diploma/Equivalent.
  • 4 years of related experience.
  • Basic understanding of the recovery principles: Hope, Personal Responsibility, Empowerment Skills, Self-Advocacy, Educational Opportunities, and the importance of using personal and community supports. Lived experience with HIV, substance use or mental health not essential but welcome. Individual does not need to be in recovery him/herself, but must be able to confer empathy through lived experiences. Peer Recovery Navigator certification or equivalent experience Excellent verbal and good written communication.
  • Commitment to a positive, collaborative work environment that serves clinic staff and providers in order to better care for patients.
  • Valid Driver’s License.
  • Flexibility with work schedule days/hours needed.
  • Ongoing commitment to safety regarding all infectious disease protocols, including flexibility with changing protocols as necessitated by the COVID-19 pandemic Some competency using a computer, including email use and Microsoft Word.

This may include documentation of having two (2) MMR vaccines; two (2) Varicella vaccines; or antibody status to these diseases from laboratory testing. Blood tests for immunities to these diseases are ordinarily included in the pre-employment physical exam except for those employees who provide results of blood tests or immunization documentation from their own health care providers. Any vaccinations required for these diseases will be given at no cost in our Occupational Health office.
