Job Summary

Job Summary
The Department of Pediatrics in the University of Utah School of Medicine has an immediate opening for a Clinical Nurse Coordinator. The Nurse Coordinator works with other team members in a team-based model of care, coordinating, planning, assessing, and evaluating care services to ensure safe and effective patient care.  Additional activities include assisting with staff development/education specific to Adolescent Medicine, quality improvement activities, and performing other duties to facilitate coordination of care for patients.
 
The Clinical Nurse Coordinator. will work with highly skilled team members to provide high value patient care in an organization focused on patient safety, patient and parent satisfaction and positive patient outcomes.  As a team, our objective is to establish and maintain effective processes and collaborative working relationships. 

Responsibilities
Responsibilities
  • Assist families and providers in navigating legislative changes and facilitating access to care with out of state providers.
  • Coordination of care with out of state providers.
  • Provide education to patients, families, and providers regarding the care of LGBTQIA+ adolescents, including gender affirming healthcare (puberty blockers, hormone replacement therapy, non-medication gender-expression care) eating disorders, and other adolescent specific conditions.
  • Work closely with the pre-authorization team and follow up with them regarding medications we are trying to get covered. Work closely to coordinate patient care with home health pharmacies and Homecare RN's that require their services.
  • Assess patient and family needs for clinical services; communicate directly and indirectly with healthcare providers to appropriately coordinate all aspects of patient care.
  • Maintain and create resources for patients and families.
  • Implement processes to improve patient care and clinic experience - patient tracking and follow-up reminders in EMR (Electronic Medical Record).
  • Assist patients in understanding how to access insurance coverage and benefits - often including calling to advocate on behalf of patients and families.
  • Reach out to community resources to advocate for patients at their schools and in extracurricular environments.
  • Participate in outreach events and educational presentations in the community.
  • Create handouts and packets to assist in legal name and gender marker changes.
  • Coordinate medication refills, letters of medical necessity, DMV forms, school care plans and medication prior authorizations as needed.
  • Coordinates quality improvement activities.
  • Refer and follow-up on referrals made to psychiatric and medical providers.
  • Coordinate admissions to higher levels of care for patients and families.
  • Facilitate urgent visits for patients outside of provider templates (involves coordination with scheduling, medical assistants, and providers).
  • Coordinate visits with multiple providers in monthly, multidisciplinary specialty clinics.
  • Schedule patient visits by acuity and provider request - often coordinating with outside treatment teams to coordinate care for patients.
  • Prepare and send patient briefs to providers prior to clinic.
  • Manage clinic flow, ensure medications are stocked and ready for patients on day of clinic and that paperwork and consents are available to patients prior to visits.
  • Perform other related duties as assigned.

Minimum Qualifications
Required
Current RQI Healthcare Provider eCredential through the University of Utah Health RQI system. The eCredential is to be obtained within 30 days of hire.

Current license to practice as a Registered Nurse in the State of Utah, or obtain one within 90 days of hire under the interstate compact if switching residency to State of Utah. Must maintain current Interstate Compact (multi-state) license if residency is not being changed to Utah.
Two years general nursing experience.

Preferred
Bachelor's degree in Nursing or equivalency (one year of education can be substituted for two years of related work experience).
One year of directly related experience in an area of specialty is preferred.
Some positions may require additional certifications such as Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), or Neonatal Resuscitation Program (NRP) within one year of hire.

Preferences
Preferences:
  • Degree in Masters of Nursing.
  • Two years of related experience in with gender-diverse youth.
  • At least one year of clinical experience with adolescent patients.
  • Prior experience caring for LGBTQIA+ adolescents, including transgender, puberty blockers, eating disorders, and other adolescent specific conditions. Familiarity with electronic medical records.
  • Demonstrated teaching, leadership, human relations, and effective communication skills required.
