Summary
The Community Health Worker (CHW) will be working in the Division of Pediatrics - Public Health on a 2-year grant. The CHW will support upSTART participants with health and safety education and programming to reduce preventable injuries. The CHW will provide individual and group-based education and support on topics including but not limited to:
- pregnancy, postpartum, and infant warning signs
- safe sleep
- child passenger safety
- fall prevention
- general home safety
- drowning prevention
Job Duties
- Supports staff and participants from upSTART (estimated to serve 250/year).
- Assists front-line staff with identification of families needing additional safety support
- Provides education on common preventable injuries
- Develops relationships with community organizations and facilitates a warm hand off to families
- Identifies and connects with community organizations for infant and maternal care supplies and resources related to safety
- Promotes upSTART programs at community events
- Collects program data and enters the data into the program and state databases
- Coordinates with the project staff to schedule visits, send reminders, and contact participants for follow-up as needed
- Documents all activities, service plans, and results
- Maintains confidentiality per HIPAA guidelines in regards to patient information.
- Attends program staff meetings.
- Performs other duties as assigned.
- High School diploma or GED.
- Two years of relevant experience in health/social services.
- Community Health Worker Certification.
- Bilingual (English/Spanish).
