Community Health Worker

Summary

The Community Health Worker (CHW) will be working in the Division of Pediatrics - Public Health on a 2-year grant. The CHW will support upSTART participants with health and safety education and programming to reduce preventable injuries. The CHW will provide individual and group-based education and support on topics including but not limited to:

• pregnancy, postpartum, and infant warning signs
• safe sleep
• child passenger safety
• fall prevention
• general home safety
• drowning prevention

The standard work schedule is Monday - Friday, 8:00am - 5:00pm, but may require occasional weekends.

Job Duties

• Supports staff and participants from upSTART (estimated to serve 250/year).
• Assists front-line staff with identification of families needing additional safety support
• Provides education on common preventable injuries
• Develops relationships with community organizations and facilitates a warm hand off to families
• Identifies and connects with community organizations for infant and maternal care supplies and resources related to safety
• Promotes upSTART programs at community events
• Collects program data and enters the data into the program and state databases
• Coordinates with the project staff to schedule visits, send reminders, and contact participants for follow-up as needed
• Documents all activities, service plans, and results
• Maintains confidentiality per HIPAA guidelines in regards to patient information.
• Attends program staff meetings.
• Performs other duties as assigned.

Minimum Qualifications

• High School diploma or GED.
• Two years of relevant experience in health/social services.

Preferred Qualifications

• Community Health Worker Certification.
• Bilingual (English/Spanish).
