Job Description

Health and Wellness Services at Fort Hays State University is seeking a 10-month position as a Case Manager. This position will help expand and strengthen FHSU’s healthcare services delivered through Health and Wellness Services through effective case management and care coordination for students who experience episodes of mental health or medical problems. These student’s academic and personal pursuits are at extreme risk without intensive cross-service collaboration between Health and Wellness Services, the Office of Students Affairs, as well as support and treatment within the community. This position will also provide campus-wide outreach services in promoting positive physical and mental wellbeing on campus.

Minimum Qualifications :
  • Bachelor’s degree in psychology, social work, counseling, or related mental health field.
  • Excellent interpersonal skills specific to mental health consultation and case management.
  • Ability to develop strong working relationships, positively represent Fort Hays State University, and interact effectively with both health/mental health professionals and non-professionals regarding mental health concerns and clinical care.
  • Previous experience working with a young adult population or in the behavioral health field.
Preferred Qualifications :
  • Master’s degree in psychology, social work, counseling, or related mental health field.
  • Licensed or license-eligible within professional field.
  • Experience providing personal counseling services.
  • Experience with crisis management.
  • Experience facilitating support groups or group counseling.
  • Experience facilitating outreach, presentations, or trainings.
  • Experience working within a multi-disciplinary team.
  • Excellent writing, organizational, and computer skills as evidenced by application documents.
Responsibilities:
  • Participate as part of the clinical team, assisting with triage and follow up care, consultation regarding campus and community referrals, case management services, and crisis intervention services.
  • Develop and implement systems for follow-up with students referred to community resources (psychiatrists, clinics, treatment programs) to verify they were assessed, successfully referred, and compliant with treatment recommendations. Schedule appointments for patients and assist in overcoming barriers to care such as transportation to and from appointments.
  • Assist with campus-wide outreach and prevention programming on relevant issues impacting college students.
  • Establish and maintain a comprehensive list of local health resources and assist students in finding health services near their homes. Cultivate relationships with off-campus mental health providers, private practitioners, hospitals and inpatient units, and other community partners.
  • Maintain records and documentation of clinical and case management services, in compliance with law, ethics and best practices.
  • Attend and participate in staff meetings and training activities. Pursue professional development related to the responsibilities of the position and profession.
  • Participate in Student Affairs’ staff meetings and serve as a member of various Student Affairs’ committees.
  • If licensed to do so, maintain a small personal counseling caseload of FHSU students, faculty, and staff.
  • When students are hospitalized, participate in discharge planning by coordinating follow-up care and facilitating access to services on- and off-campus. Assist in successfully transitioning students back into Fort Hays State University.
  • Assist in analyzing sources of financial assistance for prescriptions and other health resources.
  • Other duties as assigned.
Appointment Date: The start date will be determined after acceptance of an offer and successful completion of a background check.
