Position Description

Performs advanced level social work. Supervises lower level clinical social workers and/or ancillary staff.

Major/Essential Functions
  • Actively participates in daily WRMF plan of care meetings
  • Assists and coordinates with patients, patient’s families and other agencies to ensure continuity of care for patients releasing from prison.
  • Performs a variety of complex case management tasks
  • Communicates w/ TTUHSC medical staff and TDCJ administration the designation of Critically/Seriously ill patients and those on hospice care
  • Provides status updates to patient’s designated recipients of their PHI
  • Maintain physical, psychological and personal safety and the safety of coworkers, IP’s, security and the community.
  • Maintain current social work license and required certifications
  • Support the Values Based Culture of Texas Tech University Health Sciences Center.

Required Qualifications

Education:
A minimum of a Bachelor's Degree (B. A.) from an approved accredited four-year college or university in a behavioral science.
License/Certification: Credentialed as a Certified Social Worker.
