Position Summary
The Center of Excellence for Alzheimer’s Disease is a DOH funded program to support the Alzheimer’s Disease and Memory Disorders Center ( ADMDC ) at the University at Buffalo to provide state of the art medical services consistent with evolving practice standards which enhances and standardizes AD/D care in the region. The goal is to facilitate early diagnosis, provide assistance with care planning, educate the health care workforce and integrate special populations. The CEAD is seeking a professional to fill the position of administrative and clinical support LPN . Duties include:

- initial contact person for CEAD WNY
- phone/email intake
- directing inquiries
- assisting with reports
- supporting RN and social worker in clinic
- training health care workforce in various healthcare settings
- supporting the rural Federally Qualified Health Care clinic

- Associate degree
- 3 years of experience in patient care setting
- License (Licensed practical nurse)
- Dementia experience at least 1 year
Salary Range
$50,000 - $55,000
