CASE MANAGER (Counseling Center)

Scope of Job:
Responsible for coordinating and facilitating referral process
between students in need and support services for the Counseling
Center's grant project: UTRGV Mental Health Frontline.
Description of Duties:
• Coordinates and facilitates the referral of students in need to
various support services on and off campus.
• Assists in organizing and tracking students’ utilization of
• Provides ongoing communication with the grant project leader and
attends grant project meetings.
• Provides casework services to include the assessment of need,
formation of plan, intervention, and follow-up monitoring of
individual student status.
• Responsible for establishing relationships with students while
providing general support and individual case management services
for students receiving Counseling Center services from entry
• Interviews and screens students to gather information to assess
• Assists in the development and implementation of treatment plans
to meet students needs.
• Coordinates and monitors services, including comprehensive
tracking of students activities in relation to care plan.
• Maintains case history and progress records; prepares reports as
• Facilitates and coordinates access to campus and community social
• Conducts periodic reviews and follow-ups to evaluate quality of
clients’ referral experiences and subsequent care.

Bachelor’s degree in social work, counseling, psychology or related
field from an accredited university.
One (1) year of experience in the delivery of services and/or
support to individuals experiencing mental health issues.
Use of standard office equipment.
Needs to be able to successfully perform all required duties.
Office Environment; some travel and weekend work is required.
Excellent interpersonal communication skills. Ability to manage
multiple, competing priorities. Knowledge of community resources;
of case management principles, objectives, standards, and methods.
Ability to assess clients’ needs, to coordinate clients’ services,
to communicate effectively.
