REGISTERED NURSE (Patient Communication Center)

Scope of Job:
To provide professional nursing skills and leadership in the coordination and administration of the nursing process. Responsible for assessing, developing, implementing, and evaluating a plan of nursing care for patients in an assigned area.

Description of Duties:
Answers calls, prioritizes clinical triage calls, follows clinical protocols, and coordinates services, as needed.

Verifies patient demographic information and accurately enters the updated information into the electronic health record.

Serves as an escalation point for clinical patient issues and other POD team members requiring clinical support, and provides clinical advice based on clinical protocols and procedures.

Manages and responds to escalated patient message cases and uses clinical judgment to prioritize and accommodate patients.

Creates a positive patient experience at every encounter, attempting to independently resolve any issues or concerns of the patient at the time of the phone call, within the scope of the role.

Consistently meets productivity, schedule adherence, and quality standards as set by the Communication Center.

Utilizes all resources and guidelines at his/her disposal to effectively assess, prioritize, advise, schedule appointments, or refer calls when necessary to the appropriate medical facility or personnel.

Collaborates effectively with pharmacists, radiologists, and other healthcare professionals to facilitate comprehensive patient care.

Relays critical information for diagnostic testing or medication adjustments.

Participates in regular training and updates on clinical protocols and procedures to ensure up-to-date knowledge.

Provides guidance and support to fellow team members in handling complex clinical patient issues.

Conducts follow-up calls with patients to ensure their ongoing care and satisfaction.

Collaborates with healthcare facilities and specialists to coordinate referrals for patients requiring specialized care.

Assists in the development and implementation of new clinical protocols and procedures.

Collaborate effectively with pharmacists, radiologists, and other healthcare professionals to facilitate comprehensive patient care.

Relay critical information for diagnostic testing or medication adjustments.

Accurately documents symptoms/complaints, nursing assessment, advice provided, and patient/caller response.

Partners with other Patient Communication Center teams/PODs and respective practice clinical teams on behalf of the patient to assist with clinical concerns, medication refills, or scheduling appointments.

Maintains standards of patient care as established by nursing service and the nursing profession.

Demonstrates an awareness of the total nursing needs of the patient and is responsible for seeing that they are met.

Participates in professional nursing care of patients as required.

Administers medications and/or treatments as prescribed by the physician.

Maintains accurate medical records.

Develops and monitors specific plan of action for emergency medical procedures.

Performs treatment of emergency patients when applicable, refers emergency patients to proper physician and assists physician with treatment as required.

Supervises nursing staff on patient assessments.

Supervises and delegates patient care through staff personnel.

Develops and supervises procedures for patient scheduling, record keeping, and monitoring follow up care of patients with serious medical problems.

Initiates laboratory tests and collects various specimens and cultures.

Appropriately develops and implements clinic protocols and/or policies and procedures.

Supervision Received:
General direction from the assigned supervisor.

Supervision Given:
Direct supervision of all assigned staff.

Required Education:
Graduation from an accredited school of nursing.

Preferred Education:
Bachelor’s degree from an accredited school of nursing.

Licenses/Certifications:
Current RN licensure from the Texas State Board of Nurse Examiners.

Required Experience:
Two (2) years of experience as a licensed registered nurse.

Preferred Experience:
Two (2) years or more supervisory experience as a licensed registered nurse in ambulatory or outpatient setting.

Equipment:
Use of standard office equipment. Proficiency in the use and understanding of medical equipment such as oxygen tank operation, sphygmomanometer, phlebotomy/lab equipment/supplies, supplies for ear irrigation, thermometers, pulse oximeter, EKG machine, nebulizer, and centrifuge.

Working Conditions:
Needs to be able to successfully perform all required duties. Usual health care facility conditions with possible medical or psychiatric emergencies. Exposure to all common diseases. Must constantly consider safety risks and provide safe environment conducive to patients, visitors and staff. Work environment involves exposure to potentially dangerous materials and situations that require following extensive safety precautions and may include the use of protective equipment.

Other:
Knowledge of health care laws and regulations. Ability to provide patient care, to prepare and maintain records. Extreme accuracy in following written and verbal directors and in the dissemination and application of technical information.
