Job Description:

The role of the Pharmacist II is to provide the best prescribed pharmacotherapy to patients, supporting the
patient-centered academic mission of the University of Toledo Medical. Their responsibilities include, but
are not limited to, processing prescription orders; reviewing patient profiles; preparing, dispensing, and
managing medications to support safe and effective drug therapy for patients of all ages. Additionally, the
Pharmacist II may adjust therapy and order labs per a consult agreement. They provide education to
patients, caregivers, health care professionals, students, and the public, as well as drug information to the
medical and nursing staff as requested.

Minimum Qualifications:

Pharmacist II (U94)

Education/experience/licensing:
Professional pharmacy degree from an ACPE-accredited College of Pharmacy
Registered to practice in the State of Ohio or eligible for licensure
BLS and ACLS Certification
Board Certification in Pharmacotherapy, Ambulatory Care Pharmacy or equivalent advanced certification in area of specialty required
Residency trained candidates must obtain board certification within 12 months.
PGY1 Residency training or at least three years of applicable pharmacist experience
Meet ASHP preceptor qualifications within 12 months
May underfill with Pharmacist I for up to four years to allow eligibility and completion of board certification

Communication and other skills:
Self-directed, motivated and capable of leading supportive personnel
Able to work well with the public and other health professionals.
Possess the ability to resolve conflicts and respond appropriately to complaints.
Demonstrate effective interpersonal and culturally appropriate communication skills to provide excellent customer service.
Able to train, evaluate and supervise professional and technical personnel and staff

Graduate of an accredited College of Pharmacy
Registered to practice pharmacy in the State of Ohio or eligible for licensure

Hospital pharmacy experience preferred

Possess the ability to resolve conflicts and respond appropriately to complaints.
Able to work well with the public and other health professionals.
Provides excellent customer service
Demonstrate and utilize effective interpersonal and culturally appropriate communications

Physical Requirements:
Visual acuity of 20/20 or corrected to 20/20 including color vision when applicable
Ability to hear (with or without aides)
Ability to orally communicate
Good manual dexterity
Ability to push, pull, roll, and transfer/lift 50 pounds occasionally
Ability to stand, walk constantly
Ability to bend, reach, stoop, kneel occasionally
