Scope of Job:
To provide specialized technical skills or diversified clerical duties associated with medical records procedures.

Description of Duties:
• Maintains patient files and retrieves files for scheduled appointments, files patient charts, files patient data upon receipt of information, initiates records for new patients.
• Performs various detailed and technical tasks to input and/or retrieve /release patient information while ensuring patient health information is complete, accurate, timely, and compliant.
• Responds to requests for medical records, processes letters and reports based on established guidelines.
• Monitors data integrity, researches/merges duplicate medical records, and identifies/corrects patient overlays according to established protocols.
• Verifies and cross-references information in Health Information Management systems to ensure document accuracy.
• Prepares, scans, and indexes protected patient health information documentation in the electronic medical record.
• Timely responds to requests for release of protected patient health information by receipt via walk-in, telephone, written, and referral while ensuring the appropriate information is released.
• Assigns proper codes to patient medical records.
• Follows confidentially and HIPAA guidelines.
• Performs other duties as assigned.

Supervision Received:
General supervision from assigned supervisor.

Supervision Given:
Direct supervision of assigned staff.

Required Education:
High School Diploma or equivalent

Preferred Education:
Associate degree or one (1) year of college in Business Administration, Health Information Management or other closely related field.

Licenses/Certifications:
Preferred: Certification as a Registered Health Information Technician (RHIT).

Required Experience:
One (1) year of experience in Medical Records field.

Preferred Experience:
Associate degree or one (1) year of college in Business Administration, Health Information Management or other closely related field.

Equipment:
Use of standard office equipment.

Working Conditions:
Needs to be able to successfully perform all required duties. Office Environment; some travel and weekend work is required. Exerting up to 20 pounds of force frequently, and/or a negligible amount of force constantly to move objects.

Other:
Tact, diplomacy, discretion, and confidentiality of medical information required in all matters.
