Job Summary

The Registered Nurse is a full-time position, providing primary care as a member of the collaborative Partnership team to people living with HIV or at risk for acquiring HIV. Under the general supervision of the Medical Director, the Registered Nurse provides professional services in the physical and psychosocial health care and concerns of patients. Responsibilities include: assessing health status, performing triage, and creating plans for therapeutic regimens both via phone (triage of provider and nurse phone lines), and in person (urgent needs); following medical protocols for plans of care; educating and informing patients and partners or family on health needs, issues and treatments, and providing adherence counseling and support to patients with ongoing viremia.

Essential Functions

Clinical responsibilities such as triage and physical assessment, medical history taking, checking vital signs, counseling, and treating patients with minor illnesses and injuries; collects and interprets data in consultation with a nurse practitioner or physician; communicates findings on the medical record and with the patient to facilitate further data collection and exchange of ideas in order to arrive at a mutually agreed-upon treatment plan.
  • Must be able to assess and determine if a medical situation requires an emergency response and initiate the appropriate response.
  • Monitors, delegates and responds to triage calls on the nurse and provider lines.
  • Talks directly to patients on the telephone and directs them to the emergency room or urgent care center as needed, offers home care advice, and connects patients to the front desk to schedule appointments.
  • Acts in “Ask a Nurse” capacity, handling routine information requests from patients, e.g., “Do I need a flu shot every year? When are you giving these shots?” .
  • Conducts scheduled nurse visits with patients in need of supplemental routine follow-up care (ie. BP checks, education on medicine adherence, reconciliation, and administration (ie. Nebulizers, inhalers, self-injections), provides wound checks, hospital follow-up’s, etc.
  • Administers medications directly to patients including injections, nebulizer treatments, and oral meds.
  • Conducts MAT visits for patients with opioid use disorders (OUD).
  • Facilitates and follows up patient process to obtain medical marijuana card after certification at PCCP.
  • Monitors and works closely with patients with uncontrolled HIV with frequent check-ins either via phone or in-person. Provides adherence counseling and works to elicit barriers to treatment including mental health conditions, substance use disorders, and HIV stigma. Interacts frequently with medical case managers and the PCCP Outreach Team in order to coordinate care.
  • Ensures accurate notes of all consultations and treatments are recorded in the patients’ record via Epic Ambulatory Electronic Health Record system.
  • Provides patient education regarding disease processes, therapies and healthful behaviors.
  • Assists in completion of routine patient forms and letter generation as appropriate.
  • Communicates professionally with staff, consultants, patients, family members, and community.
  • Serves as a patient advocate in alignment with the Partnership’s mission.
  • Provides backup to Pharmacist.
  • Participates in staff meetings and actively contributes to the Committee for Quality Improvement.

Required Qualifications
  • Minimum of a bachelor’s degree in nursing or Equivalent
  • Minimum of 3 years of experience in a related field.
  • Current Certification as a Registered Nurse in Pennsylvania at the time of hire.
  • Current CPR/BLS Certification as well as proficiency in phlebotomy and injections.
  • Bilingual (English/Spanish) skills preferred.
Preferred Qualifications
  • Ability to multi-task, triage health problems, and respond effectively to medical emergencies.
  • Familiarity with an electronic health records system -- EPIC preferred.
  • Knowledge of and experience supporting diverse patient backgrounds, including race, gender, and LGBTQ+ identities, and deep interest in advocating for marginalized communities.
  • Strong leadership, interpersonal, communication and critical thinking skills are essential.
  • Must be able to consult with other medical providers for informational purposes to ensure continuity of care.
  • Must have the ability to perform physical examinations and conduct diagnostic tests, including the use of standard equipment such as otoscopes, stethoscopes, and AED’s.
  • Must be able to stand at least 25% of the day, walk within and between buildings; must be able to assist patients to walk, stand or transfer from chair to exam table, etc.
  • Must have proficiency with standard computer skills, including Microsoft Word and Zoom for virtual meetings.
