CASE MANAGER

Scope of Job:
Responsible for coordinating and facilitating referral process between students in need and support services for the Counseling Center's grant project: UTRGV Mental Health Frontline.
Description of Duties:
• Coordinates and facilitates the referral of students in need to various support services on and off campus.
• Assists in organizing and tracking students’ utilization of support services.
• Provides ongoing communication with the grant project leader and attends grant project meetings.
• Provides casework services to include the assessment of need, formation of plan, intervention, and follow-up monitoring of individual student status.
• Responsible for establishing relationships with students while providing general support and individual case management services for students receiving Counseling Center services from entry through discharge.
• Interviews and screens students to gather information to assess service needs.
• Assists in the development and implementation of treatment plans to meet students needs.
• Coordinates and monitors services, including comprehensive tracking of students activities in relation to care plan.
• Maintains case history and progress records; prepares reports as needed.
• Facilitates and coordinates access to campus and community social service resources.
• Conducts periodic reviews and follow-ups to evaluate quality of clients’ referral experiences and subsequent care.
• Establishes and maintains relationships with referring campus and community agencies.
• Serves as liaison between campus/community providers and students.
• Performs other duties as assigned.

General direction from the assigned supervisor.

May supervise assigned staff.
Required Education:
Bachelor’s degree in social work, counseling, psychology or related field from an accredited university.

None.
Required Experience:
One (1) year of experience in the delivery of services and/or support to individuals experiencing mental health issues.

Equipment:
Use of standard office equipment.
Working Conditions:
Needs to be able to successfully perform all required duties. Office Environment; some travel and weekend work is required.
Other:
Excellent interpersonal communication skills. Ability to manage multiple, competing priorities. Knowledge of community resources; of case management principles, objectives, standards, and methods. Ability to assess clients’ needs, to coordinate clients’ services, to communicate effectively.
