This professional serves as a champion, ensuring that persons facing economic insecurity benefit from the interventions that improve their personal, academic, and career outlooks. The transition navigator is a professional who demonstrates commitment to the provision of cutting-edge academic and career focused programs, and support for children, adolescents and adults who face economic insecurity.
The budget period is from 7/1/2022-6/30/2023. Future funding is contingent on the renewal of grant funds.
Minimum Qualifications

Must have the use of sensory skills in order to effectively communicate and interact with other employees and the public through the use of the telephone and personal contact as normally defined by the ability to see, read, talk, hear, handle or feel objects and controls. Physical capability to effectively use and operate various items of office related equipment, such as, but not limited to a, personal computer, calculator, copier, and fax machine. No significant climbing, stooping, kneeling, crouching, reaching, standing, walking, pushing, pulling, lifting, grasping, and repetitive motions.

Minimum Qualifications
EDUCATION AND EXPERIENCE
- Bachelor’s degree in social work, psychology, human services, or a related field preferred.
- Minimum of 2-3 years of experience performing the duties outlined above (in DC, MD or VA. Preferred).
- Minimum of 1 years of experience working with an electronic health record or similar technology related to service delivery.
- Knowledgeable in navigating social welfare systems.
- Ability to take initiative, be proactive and work collaboratively to resolve challenges
- Strong written and oral communication skills
- Proficient in Microsoft Office (Word, Excel, PowerPoint, Outlook, etc.) and electronic medical records systems
- Knowledge of theoretical orientations and evidenced-based, treatment approaches for adults, children, and adolescents
- Ability to perform detailed work independently, as well as, in the team environment.
- Customer service skills required

- Engage with referred individuals and families to triage and assess needs stemming from economic insecurity. Complete necessary engagement and assessment tools.
- Develop an individualized, comprehensive plan of care outlining personal, academic, financial, and career goals.
- Capable of devising creative interventions in a timely and goal-centered manner.
- Be familiar with local community resources to support individuals and families in Prince George’s County who require substantive assistance and navigation of resources to meet the needs related to economic insecurity.
- Serve as a facilitator, advocate, liaison, and partner working with individuals/families to connect to needed supports and services ensuring interventions meet needs.
- Develops and implements academic and career focused programs for the children and youth in the program
- Makes timely and appropriate client referrals when needed
- Prepares and maintains written records of client interactions with detail and accuracy.
- Assures clients’ experiences and records are kept confidential.
- Adheres to the NASW competencies while providing remedial services, academic strategies, direct and intensive barrier removal and workplace coaching.
- Identify and integrates services and resources to support the successful implementation of interventions.
- Identify and address gaps in services and supports which prevent the successful implementation of interventions and support individual/family with navigating barriers until resolution.
- Develop understanding of services and supports in the region and establish working partnerships where appropriate to be utilized in the continuum of support as needed.
- Maintain a working relationship with program partners and attends meetings with other providers, local and state agencies, etc.
- Monitors trends and makes informed decisions regarding service delivery for individuals and families facing economic insecurity.
- Perform other duties as required
