Provides administration for Community Health Workers (CHWs) on care coordination duties. Ensures the accuracy of information collected from CHWs is current and culturally appropriate. Helps move program efforts forward, across programs for the UH CHW initiative or affiliated projects.

  1. Prepares, organizes and executes care coordination initiatives for team members.
  2. Assists with the implementation and on-going monitoring of care coordination delivery.
  3. Facilitates hub meetings for students, UH faculty and staff to identify high/low risk participants for outreach and care coordination.
  4. Develops, organizes and coordinates continuing education classes, training and opportunities for team members, CHWs and other professionals in the community.
  5. Ensures coverage for community education programs, classes and trainings.
  6. Identifies patient compliance barriers, still ensuring quality of care coordination delivery.
  7. Helps community members manage their care through the coordination of other CHWs.
  8. Creates positive community relationships through the support of field-based CHW�s to promote trust and increase engagement in health preventing behaviors and care coordination.
  9. Assists in developing curricula to organizations in order to provide approved continuing education by an authorized provider.
  10. Maintains partnerships with community based organizations (local schools/ school districts or health and community non-profits or stakeholders in the CHWs community/neighborhood Regularly reviews data collected and corrects as needed. Manages CHW Initiative website. Prepares reports and summaries upon completion of programs, certifications and/or trainings. Performs other job-related duties as required.

Qualifications :

Community Health Worker I -
High School Diploma- no experience necessary and Health Worker Certification must be obtained within at least the last 12 months.

Community Health Worker II -
High School Diploma and 1 year of experience, and Health Worker Certification obtained within at least the last 12 months.

Community Health Worker III -
High School Diploma and 3 years of experience, and Health Worker Certification obtained within at least the last 12 months.

Education: Requires a basic knowledge of grammar, spelling, punctuation and simple mathematical functions like percentages, ratios, etc. as might normally be acquired through attainment of a high school diploma or GED.Experience: Requires a minimum of three (3) years of directly job-related experience.Certification/Licensing: Texas State Community Health Worker Certification.
