The Care Coordinator provides both clinical and clerical support within the Tilman J. Fertitta Family College of Medicine. Ensures the provision of quality and compassionate evidence-based care in a highly integrated environment. Accountable for collaborating with the care team to consistently communicate and document high-risk patient profiles while ensuring continuity of care for all patients. Works closely with the care team to connect the dots of collaborative patient care.

  1. Accepts phone calls from patients forwarded by the front desk involving a clinical inquiry specific to existing patients.
  2. Performs protocol-based triage as appropriate; route phone calls in the form of phone notes documented in the EMR to the provider for review and instructions; and return calls to patients as directed by the provider in coordination with the Clinical Nurse Manager.
  3. Places phone calls or creates template-based letters, as directed by the provider, in regards to all lab results for delivery to the patient in a timely manner in coordination with the Clinical Nurse Manager.
  4. Reviews schedules on a daily basis to ensure that all pre-visit preparations are completed prior to the patients visit, ensures that all lab and imaging results, consultation reports, care transition details, or major changes in health status are available in the patients medical record.
  5. Calls patients, consultants, imaging centers, and/or hospitals to arrange for timely delivery of pre-visit preparations for patients.
  6. Refills prescription medication per the standing order protocol.
  7. Initiates external specialty or partner referrals to assist in the continuum of care.
  8. Coordinates high need patients to receive the specialty or partner care they require.
  9. Assists the care team in the completion of reports that allow the team to assess and manage the health needs of the patient population, such as defined groups of patients (e.g., patients with specific clinical conditions such as hypertension or diabetes, patients needing tests such as mammograms, immunizations, etc.)
  10. Performs other job-related duties as assigned.

Qualifications :

Experience: Requires a minimum of three (3) years of directly job-related experience.

Education: Requires specialized training in basic trades, principles, procedures practice, routines or techniques in a specific area or trade which might normally be acquired through up to 18 months of education or training beyond the high school level. Vocational competence in the operation of mechanical or electronic equipment may be required.

Certification/Licensing: Requires technical certification or registration directly related to the job, as specified on the job posting details. National Medical Assistant Registration/Certification or Practical Nurse License preferred.
