The Community Health Worker ( CHW ) for the Advance Health Equity Program (Advancing Equity by Building Community Infrastructure through Community Health Workers) works closely with the Advance Health Equity program leaders, TSU -based programs, Faith/community-based organizations, Federally Qualified Health Centers, clinics, or health-care systems addressing health disparities to provide support, education, coordination, and resource connection for community individuals to promote health equity.
The main goal of the Advance Health Equity Program is to advance health equity in communities impacted by COVID -19 pandemic through COVID -19-related and other health priority promotional activities conducted by community health workers (CHWs). This aligns with Centers for Disease Control and Prevention ( CDC )’s mission to reduce COVID -19-related health disparities and promote health equity among racial/ ethnic minority population. It also aligns with Houston Health Department ( HHD )’s goal to build community infrastructure in health promotion to minority communities through CHWs. Under the direct supervision of the Advance Health Equity Program leadership team to perform duties related to the operations, functions, and activities of the project.

DISTINGUISHING CHARACTERISTICS :
The community health worker ( CHW ) serves an important function of connecting the underserved community individuals with education, medical screening, resources, and treatment. The CHW is a liaison that provides culturally competent health and social services to the community and has a close understanding of (or able to become knowledgeable about) the ethnicity, language, socio-economic status, and life experiences of the community served.

This is a Grant Funded position. Essential Duties Summary
Health promotions, Community Engagement, and Outreach (~50%)
  • Cultivate and maintains relationships to advance health equity
  • Provides health equity related services to individuals living in communities impacted by COVID -19 pandemic
  • Connects individuals living in communities impacted by COVID -19 pandemic with resources and health promotions related services
  • Collaborate with stakeholders on and off campus to provide a comprehensive, multidisciplinary services to promote health equity for ethnic minority population
  • Becomes knowledgeable about periodic community surveys that 1) provide baseline and follow-up health data, 2) identify program needs and priorities, 3) evaluate the effectiveness of programs and services, and 4) track community health status and behavior
  • Incorporate cultural competence, multicultural diversity and social justice concepts and principles into Health promotions, Community Engagement and Outreach services
  • Knowledgeable (or able to become knowledgeable) about clinical research participation and advocates for diversity and representation in clinical research/trials
  • Prepare materials, set-up, plans, implements, and participates in outreach events, seminars, conferences, and networking activities
  • Serves as direct contact for the community by phone, email or website inquiry for all Advance Health Equity programming

Administrative (~40%)
  • Ensure day to day operations of the Advance Health Equity Program
  • Vets and updates all information shared through the Advance Health Equity Program
  • Maintain accurate event calendar and communicate event details with the team
  • Facilitates communication and coordinates services between the team, sites and the community
  • Prepares meeting agendas, reserves rooms and facilities, and distributes minutes of meetings when required
  • Answers routine questions concerning the Advance Health Equity Program activities and operations
  • Types and edits routine correspondence, reports, and other documents from rough draft to provide final typewritten copy for review and signature
  • Makes copies of correspondence or other printed materials and operates a variety of standard office equipment
  • Compiles all data required for progress report
  • Organizes and maintains file system. Files correspondence, materials and other records for future reference according to established file retention schedule and procedures
  • Coordinates and tracks pending assignments or requests from internal and external contacts
  • Verifies information or records and ensures accuracy and completeness
  • Work with student interns, volunteers, and/or peer educators to achieve the goals of the Advance Health Equity Program
  • Report directly to the Advance Health Equity Program Director
  • Works closely with Advance Health Equity Program Director and other key personnel to prepare reports and deliverables to funding agency
  • Willingness to work flexible hours to meet the needs of the program
  • Performs other job-related duties as assigned

Financial (~10%)
  • Orders and maintains supplies and outreach materials for the Advance Health Equity Program
  • Tracks Advance Health Equity Program expenditure

Associate degree with a minimum of 3 years Community Engagement/Outreach or Health Promotions or high-school diploma/ GED certification with a minimum of 6 years Community Engagement/Outreach or Health Promotions similar experience required.

Most relevant degree areas include public health, health promotion, health policy, social work-related degree, or health education. Degrees in other fields may be considered with evidence of sufficient university-level coursework and/or professional continuing education in the areas listed above. Required Licensing/Certification
Valid Texas Community Health Worker ( CHW ) license required. Knowledge, Skills, and Abilities
Knowledge of:
  • Policies, procedures, and practices.
  • Knowledge of basic office and financial processes and procedures

  • Demonstrated skills in a variety of educational methods including presentations, workshops, trainings, community engagement/outreach, and small group discussion/facilitation
  • Demonstrated skills in a variety of computer software applications (e.g., word processing, excel spreadsheet, PowerPoint, and statistical software)
  • Ability to work independently while collaborating with other team members and exercise sound judgment

  • Excellent communication, both verbal and written, with strong public speaking skills
