Care Coordinator

About the Opportunity

The Care Coordinator serves as a front-line staff working with student, faculty, staff, parents and community members on medical leave of absence, re-entry, referrals, resources and education. This position requires the ability to manage a high volume of outreach, communication and competing priorities with all levels of the organization.

The Care Coordinator is front facing role in the medical leave and re-entry process for university students across the global network. The position is responsible for reviewing medical leave and re-entry requests, clinical documentation, and evaluating if the care delivered meets the university standards. Partnering with global campuses, specialized focus on international student management and compliance are areas of focus for this role. In addition, the Care Coordinator will manage a case load, providing compassionate and clear communication and outreach, while maintaining accurate and timely documentation, while working closely with Director of Coordinated Care and Health Promotions.

This role is responsible for maintaining a reliable and vast referral database, coordination with student referral resources as well as knowledge in health insurance plans. The Care Coordinator ensures attention to insurance coverage for care resources and tracking of students. The ideal candidate is a collaborative team-player responsible for carrying out a wide variety of tasks within and outside of their assigned portfolio and contributes to team projects as requested or assigned.

This position is eligible for hybrid work.

MINIMUM QUALIFICATIONS:

Bachelor's degree preferred. Two years' experience in case management or health care setting.

Ability to handle a high level of sensitive and confidential information, with attention to details while maintaining confidentiality.

Excellent written and verbal communication skills

Demonstrated familiarity with mental health and medical clinical practices and services.

Strong proofreading, attention to detail, and follow through skills are essential.

Strong organizational, problem solving, and time management skills, with the ability to manage multiple priorities.

Ability to establish relationships with various community constituents: students, families, staff, faculty and clinicians.

Experience with electronic medical record systems and database management (eClinical Works preferred), Microsoft Office, especially Excel.

The ideal candidate should demonstrate empathy, compassion and understanding.

KEY RESPONSIBILITIES & ACCOUNTABILITIES:

Medical Leave and Re-entry:

Manages a case load of students in Medical Leave and Re-entry process

Responsible for responding to communications received via the central email box, phone line and faxes, with the expectation of a timely, accurate and compassionate response with attention to detail.

Maintain various electronic records including medical record and academic record.

Reviews clinical documentation, communicates with all constituents regarding university standards, care expectations and delivery, approvals and administrative matters.

Tracks case load and provides related reports at daily team check-ins and on-going staff meetings.

Referral Scheduling and Coordination:

Coordinates timely appointments for students with the community specialists including verifying insurance coverage.

Maintains close working relationship with colleagues at referral practices and within UHCS.

Tracks appointments and receipt of information from specialists and students.

Answers questions and supports students with clear communication and follow-up support as needed.

Administrative:

Supports the administrative needs of University Health and Counseling.

Provides guidance to students regarding connecting to resources on campus and off

Develops resource materials to support the education of students.

Actively participates in community wide events, including tabling and connecting with external resources

Participates in special projects as requested by supervisor.
