Job Description:

The Clinical Pharmacist-Solid Organ Transplant Quality Coordinator at The University of Toledo Medical Center (UTMC) works closely within the multidisciplinary team as the medication expert aiding in managing the complex pharmacotherapy regimens of solid organ transplant recipients in both the acute care and ambulatory care settings. Responsibilities include assisting in maintaining compliance with requirements of external agencies specifically related to Transplant including CMS, UNOS, Ohio Department of Health (ODH), Joint Commission and ESRD Network. The Clinical Pharmacist-Solid Organ Transplant/Quality Lead precepts, mentors, and educates students, residents, pharmacists, and other health care professionals in addition to maintaining documentation related to transplant pharmacy activities that are compliant with CMS regulations.

Minimum Qualifications:

1. Graduate of accredited pharmacy school.
2. Must hold current and unrestricted Ohio Pharmacist License.
3. PGY1 residency or 3 years of clinical pharmacy practice required.
4. PGY2 Residency in Solid Organ Transplant or equivalent experience in solid organ transplant (3-5 years) without a PGY2 Residency preferred.
5. Board certification or specialty certification preferred.
6. Strong computer skills required.
7. Knowledgeable in physical, chemical, and pharmacological aspects of pharmacy practice with special interest and ability to develop and implement innovative pharmacy practice models and programming.
8. Demonstrates effective and culturally appropriate communications.
9. Utilizes effective communication to provide excellent customer services and a positive public image.
10. Must be able to withstand pressure due to work volume, interruptions, pace, job variety with an expected degree of accuracy
