We are seeking motivated and empathic individuals who are eager to learn and are passionate about helping others as a Community Health Worker. This role is a frontline public health worker who is also a trusted member of the community and/or has a close understanding of the community served. This trusting relationship enables the Community Health Worker to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.


The Community Health Worker will be responsible for helping patients to navigate and access community resources, various social services, and the healthcare system. The Community Health Worker will be expected to build and maintain relationships with study participants, including reaching out to them on a regular basis via telephone, at the clinic site, or participants' homes or agreed-upon places in the community. This will occur during specific follow-up visits and on an as-needed basis. They will be provided with appropriate training and oversight.

Specific Duties & Responsibilities

  • Providing education on how to manage BP (Blood Pressure) through self-monitoring and healthy behaviors, including diet and exercise.
  • Reinforcing positive Blood Pressure self-management behaviors through follow-up encounters.
  • Provide training and reinforcement on the correct use of home blood pressure cuff, and the use of a mobile health app.
  • Linking participants to existing clinical and administrative services.
  • Linking participants with community resources to address health-related social needs.
  • Assisting patients with the use of the mobile health application (Sphygmo).
  • Document interactions with patients including information needed for research purposes . Since this study is geared towards improving blood pressure control, we want CHWs who are flexible, organized, and patient-centered.

Special Knowledge, Skills, & Abilities
  • Valid Maryland, Virginia, Delaware, or Washington, D.C driver’s license and access to an insured automobile that lists employees on the insurance policy.
  • Familiarity with Microsoft Office applications (i.e., Word, Excel, Outlook).
  • Attention to detail.
  • Ability to deal with frequent changes, delays, and/or unexpected events.
  • Ability to establish positive, supportive, and trusting relationships with and among patients, their family members, and colleagues.
  • Ability to work collaboratively and effectively within a team.
  • Ability to take the initiative, problem-solve, and make decisions.
  • Having empathy and compassion for those struggling with their health and issues in their lives.
  • Reliable and dependable.
  • Strong oral and written communication skills in English.
  • Strong oral and written communication in Spanish is preferred but not required.
  • Strong organizational and effective time management skills.
  • Willingness to advocate for patients in the healthcare setting.
  • Willingness to learn new things (such as new computer programs, i.e., REDCap; or new skills).


Minimum Qualifications
  • High School Diploma or GED.
  • At least two years related experience with community engagement and/or health (i.e., medical assistant, classroom assistant, CHW, outreach worker, volunteer, etc.).
  • Additional education may substitute for required experience, to the extent permitted by the JHU equivalency formula.
