Job Summary

The Health Education Coordinator will work and oversee all aspects of the outreach, health promotion, and disease prevention training programs. The Health Education Coordinator will work with the CNHP Community Wellness HUB Executive Director and staff to ensure programs are implemented and aligned with the mission, vision, and values of Drexel University, CNHP Community Wellness HUB, and the Dornsife Center. The Health Education Coordinator develops and maintains a tracking and reporting system of the organization’s interactions within Drexel University, its academic programs, and its community partners’ referral sources. This person will establish and maintain linkages with community health and social organizations in the West Philadelphia area. The successful candidate will have a strong desire to serve in the community and have a background in community health, community engagement, community outreach, or other socially engaged disciplines. The Health Education Coordinator will report to the CNHP Community Wellness HUB Executive Director.

Essential Functions
  • Assesses the health needs of community members in the Promise Zone neighborhoods.
  • Assists with providing health promotion, disease prevention, and treatment education activities in community settings and at the CNHP Community Wellness HUB.
  • Creates and implements strategies to contact community members, organizations, and health care providers to assist and promote seamless screening, referral, and treatment appointments.
  • Assists with developing health education sessions and material for health promotion education, disease prevention, and follow-up treatment.
    • Develops and maintains programs that assist individuals, families, populations, and communities in maintaining healthy lifestyles.
    • Designs and implements linguistically and culturally appropriate health promotion and disease prevention messages and educational programs, workshops, or 1-1 sessions. o Prepare and distribute health education materials, such as reports, bulletins, flyers, newsletters, posters, and visual aids.
    • Collaborate with CNHP faculty, health specialists, and civic groups to determine available community health needs and services and develop goals for meeting needs.
    • Build partnerships and promote guidance to agencies and organizations on health assessments of health education needs and developing and delivering health education programs.
  • Assists in facilitating community health education workshops at the CNHP Community Wellness HUB, the Dornsife Center, and community meeting spaces.
    • Utilizes program planning methodology based on the community’s voices, screening, referral, and treatment to promote their health, wellness, and optimum outcomes in various community and faith-based settings and at the CNHP Community Wellness HUB.
    • Develops and maintains health education libraries to provide resources for staff and community agencies.
    • Coordinates the logistics of all educational sessions.
    • Designs and implements various health education training programs for the CNHP Community Wellness HUB staff to build the capacity of the team to address community health concerns.
    • Prepare all training materials.
    • Documents all educational workshops and 1-1 sessions, location, time, number of participants, outcomes, and persons assisted.
    • Create and monitor the maintenance of databases, mailing lists, telephone networks, and other information to facilitate the functioning of health education programs.
    • Monitors all staff implementing community education programs.
    • Designs and conducts evaluations to assess the quality and performance of health education programs.
  • Coordinates health care services and social support of community members.
  • Attends and participates in community activities, meetings, in-services, educational programs, and other activities as deemed appropriate.
  • Assists with cultivating new relationships and partnerships with community members and groups.
    • Develop and maintain positive working relationships with agencies, organizations, community leaders, and residents.
    • Assist with creating and implementing outreach and community engagement strategies to contact community members and leaders to implement education programs and workshops.
  • Contributes to the assurance of a positive working environment.
  • Delegates with a willingness to step in as needed.
  • Prepares monthly reports on CNHP Community Wellness HUB activities.
  • Advocates for improved health resources, policies, procedures, and services that promote health.
  • Other duties as assigned.
Required Qualifications
  • Bachelor’s Degree in Public Health or related field
  • 3+ years of relevant experience
  • Previous experience in managing programs or projects required.
  • Has a strong desire to serve in the community and background in community health, community engagement, community outreach, or other socially engaged disciplines.
  • Must be flexible and adaptable as programming necessitates.
Preferred Qualifications
  • Certified Health Education Specialist (CHES) credential preferred.
  • Has proficient organizational skills.
  • Possesses good social skills with the ability to engage with community residents, organizational leaders, health professionals, and CNHP Community Wellness HUB staff.
  • Knowledge and experience working in community-academic settings with a direct understanding of challenges and issues facing urban communities.
  • Demonstrates cultural humility and compassion for community members.
  • Has good problem-solving, communication, and conflict resolution skills.
