Job Summary

The Dorothy Mann Center for Pediatric and Adolescent HIV (Section of Immunology) at St. Christopher’s Hospital for Children is seeking an enthusiastic Medical Case Manager (MCM) to provide clinical services to patients and families of The Dorothy Mann Center (DMC). Responsible to the HIV/AIDS Program Director and Social Work Program Manager for provision of comprehensive case management services, particularly family centered case management, to HIV infected and affected children, adolescents and their families. Engages in education of families, employees and community agencies and participates in the implementation of research initiatives.

Essential Functions
  • Social Work Intervention with Patients Families – The social worker assesses, plans, implements professional interventions based on knowledge of social work practice, human behavior and stages of development, social environment, and child and family development. Interventions are directed to the coping needed by patients and their families around illness and disease, hospitalization and continuing care that may be required over time
  • Documentation of Social Work Services -The social worker is responsible for accurate documentation of services in the patient record. All documentation is completed in a timely manner. The worker coordinates multidisciplinary/clinic rounds and patient care conferences as needed
  • Leadership Initiative -The social worker demonstrates leadership in their programs, the department, hospital and, as appropriate, the community
  • Teamwork / Collaboration -Demonstrates consistent team building skills regarding conflict resolution, stress management, liaison with other departments and staff advocacy. Fosters a collaborative relationship with departmental colleagues, including a consistent willingness to respond to requests to assist in other service areas. Refrains from being critical about others and utilizes effective communication skills and lines of communication. Adequately prepares colleagues to take over responsibilities during absences
  • Appropriately participates in clinical rounds, case presentations, and staff and team meetings. Demonstrates ability to interact with interdisciplinary teams, to promote social work role with other staff and to provide consultation appropriately. Carries an appropriate caseload and demonstrates a receptivity to accepting new assignments.
  • Dependability: Effectively manages use of time and maintains high level of work performance. Displays a thorough/accurate work ethic. Adheres to absence/lateness policy. Observation of department/medical center policies and procedures. Completes all mandatory education/competencies in timely manner (i.e. HIPPA, fire safety and ethics training
  • Adaptability: Performs in new situations. Maintains composure under stress. Takes initiative/self-starter. Utilizes appropriate chain of command. Performance during periods of departmental / organizational change. Demonstrates an understanding of staff/scheduling needs and is willing to be flexible with coverage
  • Provide approximately 12 hours of medical case management services to the MYELO clinic in the Developmental Pediatrics Department
  • Assist and support outreach and recruitment initiatives, in addition to educational programs as necessary
  • Must be professional and possess the ability to communicate effectively orally and in writing
  • Able to work with diverse populations (clients and staff); sensitive to cultural, economic, gender, and sexual orientation differences
Required Qualifications
  • Master’s Degree in Social Work
  • 2 years of Experience
  • LSW
Preferred Qualifications
  • Master’s preferred from an accredited by the Council on Social Work or related field
  • Two years of paid professional experience in a clinical setting, preferably pediatric
  • One year of experience in HIV or the ability to demonstrate HIV knowledge
