Responsible for reading patient charts to determine medical history, including diagnoses and treatment given. Using a set of established medical codes transcribe patient history used by both health care providers and insurance companies. Maintains protected health information (HPI) documentation in accordance with Health Insurance Portability and Accountability Act (HIPAA).

  1. Review patients records for timeliness, completeness, accuracy, and appropriateness of data.
  2. Read and analyze patient records to determine the correct coding, used to bill insurance providers.
  3. Assign appropriate diagnoses and procedure codes for patient care, population health statistics, and billing purposes.
  4. Organize and maintain data for clinical databases and registries.
  5. Utilizes CPT, ICD-10-CM and HCPCS Level II code sets.
  6. Use classification software to assign clinical codes for insurance reimbursement and data analysis.
  7. Electronically record data for collection, storage, analysis, retrieval, and reporting.
  8. Maintain confidentiality of patients records.
  9. Work as a liaison between the healthcare providers and billing offices.
  10. May provide training on medical coding to staff.
  11. Performs other duties as assigned.

    Certificate: Certified Professional Coder CPC

Qualifications :

High school and 1 year experience

Requires a basic knowledge of grammar, spelling, punctuation and simple mathematical functions like percentages, ratios, etc. as might normally be acquired through attainment of a high school diploma or GED.Requires a minimum of one (1) year of directly job-related experience.

Additional Preferred Qualifications : knowledge of opthalmology/optometry billling and coding.

Additional Posting Information:

This position will be responsible for the daily coding of claims.
