Essential Functions (Job Duties)
Provides case management services for people living with Human Immunodeficiency Virus within our Wayne State University Adult Human Immunodeficiency Virus Program. Participates in the creation of care plans. Guides and monitors clients through the full patient care cycle from care planning to case closure. Assists clients in accessing social work, counseling and other community-based resources. Works collaboratively with other staff, ensuring a holistic approach to treatment/services is followed for each client. Connects with community agencies servicing the Human Immunodeficiency Virus population to make transitions and access easier for clients. Serves as the Lost to Follow-up Liaison within the program. Reports to Medical Director.
Provides case management services and assistance with navigating the health system to individuals infected with or affected by Human Immunodeficiency Virus/Acquired Immune Deficiency Syndrome. Increases identification of Human Immunodeficiency Virus through testing of adults-at-risk. Decreases barriers to access of care by facilitating psychosocial needs assessments to identify potential barriers to care. Works with clients to provide resources needed to eliminate or reduce barriers to treatment that may include, but are not limited to, transportation, housing, insurance, and education.
Provides care coordination, data management and acts as liaison with other human immunodeficiency virus /Acquired Immune Deficiency Syndrome organizations providing services to target population. Develops and maintains positive working relationships and partnerships with other social/human service providers as it relates to the Wayne State University Adult Human Immunodeficiency Virus Program.
Conduct intake and follow-up for human immunodeficiency virus infected and affected persons requesting services from the Wayne State University Adult human immunodeficiency virus Program which includes medical and mental health care. Develop individual service plans and adjust plans as needed. Monitor status and needs of clients through regular contact with clients to assure changes are addressed as per standards of care.
Assesses mental health status and monitors functioning. Provides ongoing supportive counseling to address mental health needs and substance use disorders including adjustment, identity, depression, family, and relationship issues. Assists clients with strategies to head off crisis and encourage stability. When necessary, communicates with friends, families and/or healthcare professionals to facilitate the continuum of care or provide additional support/resources.
Provides hands-on support in accessing resources and services including completing paperwork to obtain insurance, housing assistance, dental appointments or Supplemental Security Income. Arranges transportation services for medical appointments and other services as needed. Advocates for services on behalf of clients for identified needs and eligibility for services including food, housing, medication, educational, and vocational services.
Participates in collection and reporting of data documentation of internal and external reports as mandated by funding and the Wayne State University Human Immunodeficiency Virus Program policies. Maintains strict confidentiality of all client related data in accordance with federal, state and local confidentiality laws. Attends multidisciplinary team, staff and grant related meetings.
Performs other related duties as assigned.
Unique Duties
Qualifications
Bachelor's degree
Requires a Bachelor's degree in Social Work, Counseling, Psychology or a related field from an accredited college or university.
Experienced (minimum 2 years of job-related experience)
Requires two or more years of medical case management work experience.
Experience working with Human Immunodeficiency Virus population is strongly preferred. Training in counseling and substance use disorders is required. Must complete state-sponsored Human Immunodeficiency Virus prevention counseling and testing training/certification within 3 months of hire. This position is a combination of a social worker, clinic-based case manager and community-based case manager.
Knowledge, Skills and Abilities:
Can apply effective interpersonal skills to provide service. Can respond effectively to most sensitive inquiries or complaints.
Self-directed and able to make decisions and organize work.
Can prioritize and plan work activities. Can adapt to changing conditions.
Ability to deal with frequent changes, delays, or unexpected events.
Accepts responsibility for own actions. Treats others with respect and consideration regardless of their status or position.
Works well independently and with other clinical team members.
Ability to provide services to gay, lesbian, bisexual and transgender clients.
Basic knowledge of Microsoft Office.
