The Department of Medicine seeks aClinical Nurse Coordinator who will report to the Palliative Care Director and will function as the lead clinical coordinator of the Johns Hopkins Bayview Palliative Care Program. This position requires a candidate with strong initiative who works with minimal supervision with strong computer and organizational skills. Capable of frontline management and triage of patient/family phone calls with tact, courtesy and good judgment.

The Clinical Nurse Coordinator will serve as nurse liaison, disseminating palliative care education and practices through projects and meetings. This individual will collect and organize data for quality improvement/research projects. The incumbent for this role would need to address a wide range of clinical coordination duties, including patient- and family-centered group meetings, clinical triage of new cases, communication with other medical teams, scheduling of team conferences, and coordination for academic projects. The position will require autonomy, sound judgment and emotional intelligence, and strong organizational and interpersonal skills.

Specific Duties & Responsibilities:

  • Serve as liaison to hospital nursing staff and leadership, advocating for Palliative Care program and principles.
  • Interfaces between the program and JHBMC trainees to manage rotations, curricula, and clinical best practices.
  • Coordinates the process for safe and efficient navigation throughout the continuum of care for the patient, including coordinating communication with outpatient providers, assisting with disposition arrangements, post-discharge phone calls, sending bereavement cards.
  • Help develop, monitor and manage the processes and related policies and procedures applicable to running a state-of-the-art program.
  • Collaborate with physicians and clinicians to develop, implement and monitor palliative care initiatives based on evidence and best practice.
  • Provides primary nursing care to program patients. Primary nursing begins with the first encounter with the patient and continues throughout the course of care.
  • Leads and coordinates educational projects for a variety of learners.
  • Triages communication to/from medical teams to help coordinate medical care.
  • Collects, organizes and analyzes data for quality improvement and research projects.
  • Assist in the coordination and design of research studies.
  • Manage phone calls to/from patients/families, addressing concerns and triaging appropriately.

  • Coordinate team meetings across entities including SOM Departments and JHM Hospitals.
  • Manage new program recruits and their onboarding processes.
  • Patient experience liaison to act as point of contact and problem solve concerns or direct to correct office in complex organization.
  • Represent the Palliative Care Program at both internal institutional committees and meetings (both JHBMC and JHHS) and external conferences and professional associations.
  • Participate in family meetings on as-needed basis.

Minimum Qualifications:

Individual must be a registered nurse, licensed in the State of Maryland or state where practicing. Bachelor's degree in nursing or related discipline preferred. Current American Heart Association or American Red Cross CPR certification required. Must maintain current licensure and certification during duration of employment. Acceptable completion of the JHH credentialing process.

Preferred Qualifications: Experience with palliative care eligible patients preferred.

Additional Knowledge, Skills & Abilities:

  • EPIC experience, strong oral and communication skills, and experience with data entry and analysis.
  • Proficiency with Microsoft Office Suite and Zoom.

Physical Requirements:

Sitting, standing and walking for extended period. Reaching by extending hand(s) or arm(s) in any direction. Finger dexterity required to manipulate objects with fingers rather than with whole hand(s) or arm(s). Ability to move standard equipment through a hospital or clinical environment.

Working Conditions: Telecommuting hybrid according to divisional/departmental return to campus plan.
