The Department of Surgery seeks aCommunity Outreach Program Advisor who will interface with the breast imaging and surgical team of the breast center by providing navigation and patient education for patients undergoing diagnostic evaluation and testing of breast abnormalities. This position provides clinical functions as required in support of breast center services and programs and participates in maintaining a safe, cost effective quality of care patient environment. This position will focus on providing education, support and continuity of care for patients undergoing diagnosis and management of breast abnormalities. This individual will work closely with the breast imaging radiology team as well as with the surgical nurses and surgical faculty in supporting and navigating patients from the point of biopsy, to their surgical treatment and any additional treatments needed. This role will assist in the coordination of care directed by the patient’s providers and assist in scheduling of tests. This individual will also assist the new patient appointment scheduling office with triaging of new patients to ensure the patient is appropriately scheduled with all the disciplines needed to make a clinical treatment plan/decision making at the time of her initial consultation.

Other significant roles will include conducting breast cancer awareness programs (outreach) in the community setting. This person will also perform data collection of specific clinical quality measures and service standards which is part of the performance improvement measures for the center. This individual will also be responsible for oversight and execution of each of retreats held for patients as part of the psychological support the center provides to breast cancer survivors who have recently completed treatment as well as for women battling metastatic disease and their caregivers supporting them. This individual will assist in grant submissions related to requests for funding for breast center events as well as research related to quality of life research grant requests. This person will have an active role in quality of life research as well.

Specific Duties & Responsibilities:

  • Provide psychological support to patients having diagnostic evaluation in the breast imaging area of the Breast Center.
  • This can be accomplished by coordinating the survivor volunteer members coming on site for this type of patient support as well as personally when available by providing the support when a survivor volunteer is not available.
  • Provide clinical and managerial oversight to the survivor volunteers coming on site to support patients in imaging and assume responsibility for scheduling their onsite volunteer activities.
  • Educate patients about interpretation of pathology results from biopsies.
  • Reinforce value of screening mammography, clinical breast exam and breast self exam.
  • Perform a barrier assessment as part of the evaluation of the patient, for those who will need surgical management following biopsy and provide this information to the nurse who will be directly involved with her surgical management and clinical care.
  • Serve as navigator for patients by working closely with the providers in the breast center and performing follow up and quality control regarding the scheduling and outcome of additional imaging studies ordered.
  • Review and assess medical record information on newly diagnosed breast cancer patients prior to their scheduling/arrival for this first consultation with the multi-disciplinary clinical team to triage the patient and determine the appropriate scheduling needs based on clinical findings known thus far. (ie, when to request scheduler to arrange for patient to have a plastic surgery consultation with the breast surgeon and/or a radiation consultation at the time the patient is seen by the surgeon; when to arrange for a medical oncologist consultation for neoadjuvant chemotherapy considerations; when to arrange for a genetics consultation.)
  • Prepare the patient in advance what to expect at the time of the surgical consultation and communicate important clinical and psychosocial findings to the surgical nursing team who will be involved in the patient’s care.
  • Perform community education and outreach programs to raise awareness of breast cancer, promote mammography, clinical breast exam and how to perform a breast self exam.
  • Serve as the facilitator and coordinator for patient retreats held throughout the year; these include: fall survivor retreat; spring survivor retreat; retreat for women and their spouses battling metastatic disease; retreat for mothers and daughters when the daughter has metastatic disease; retreat for patients with metastatic breast cancer and her female care partners.
  • Serve as the coordinator and team leader for health fairs and other community education outreach programs locally and regionally.
  • Assist in submission of grants to support events, patient education, and quality of life research.
  • Other duties as assigned may include attendance at tumor board meetings, staff meetings, matching newly diagnosed patients to a survivor volunteer for one on one support.

Minimum Qualifications (Mandatory):

  • Bachelor’s Degree in public health, teaching and education or related field required.
  • Three (3) years’ related experience, including prior in a breast center performing patient navigation and other similar functions.
  • Additional education can be substituted for experience or additional experience may be substituted for education requirement to the extent permitted by the JHU Equivalency.

JHU Equivalency Formula: 30 undergraduate degree credits (semester hours) or 18 graduate degree credits may substitute for one year of experience. Additional related experience may substitute for required education on the same basis. For jobs where equivalency is permitted, up to two years of non-related college course work may be applied towards the total minimum education/experience required for the respective job.

Preferred Qualifications:

  • RN license preferred.
  • Certification as a Navigator preferred.

Special Knowledge, Skills & Abilities:

  • Experience as a team leader/managerial skills.
  • Must be able to multitask well and work efficiently with a diverse patient population and diverse faculty.
  • Must possess a solid knowledge base of breast cancer diagnosis and treatment.
  • Must have excellent communication skills and be able to multi-task.
  • Must be proficient in Word and Excel and other computer operations.
  • Must be able to work a relatively flexible schedule to accommodate the special needs of breast cancer patients seen and treated here.
  • Must be skillful in problem solving and being proactive in anticipating patient needs, taking appropriate course of action, using approved guidelines, protocols and pathways.
  • Experienced in protocol/pathway driven environment; able to deviate where appropriate and make referrals where warranted.
