Position Summary and Responsibilities
Under the general direction of the Director of Case Management, the Case Manager provides direct Case Management Services to students who are dealing with challenges relating to medical health, mental health, financial concerns, family challenges, or social/adjustment issues. The Case Manager works closely with key offices to identify and intervene as early as possible with students in need for support services. The Case Manager will also support the activities, programs and services associated with the functions of the Case Management, Advocacy, Resources, and Education (CARE) Office. Although the case management role is a non-clinical role within the CARE Office and the area of Student Living and Wellness, the individual hired for this position would need to have clinical knowledge and experience in the provision of case management services. This position has an on-campus housing option.
Responsibilities:
  • Provides direct Case Management Services to students experiencing various levels of distress including but not limited to: medical health, mental health, financial concerns, family challenges, or social/adjustment issues. This includes students experiencing high acuity concerns such as suicidal ideation and self-injury or other issues affecting their educational success. Provides support to students in connecting to on and off campus resources and assisting with the referral process.
  • Assists with the clearance process for students returning to college after hospitalizations, and communicates with the student's providers who are supporting the student in creating a treatment plan. Connects students to treatment providers in the community if students are not already connected to care.
  • Serves as a support and potential resource for students looking to establish long term care for their mental and/or physical health with an off-campus resource.
  • Assists in the coordination of the in-take process for student related issues that come to the Case Management, Advocacy, Resources, and Education (CARE) Office. Communicates with the Director of Case Management, Associate Dean of Student Living and Wellness, and other campus colleagues to track and resolve student concerns.
  • Engages with colleagues in various areas within the College such as Dean of Studies Office, Counseling Services, Health Promotion Education, Accessibility Educational Opportunity Program, Financial Services, Residential Life, Student Advocacy and Violence Prevention, Health Services and others, to advocate on behalf of students and to seek resolutions for financial, academic, health, emotional, and psychological concerns faced by students.
  • Works closely with the Director of Case Management to manage students of concern cases that require the undertaking of initial information gathering, developing recommendations for response, taking the initiative to mobilize appropriate responses and/or services, and provides information to the student, student's family as appropriate and conveys information to other areas within the college including Dean of Studies Office, Counseling Services, Health Promotion Education, Accessibility Educational Opportunity Program, Financial Services, Residential Life, Student Advocacy and Violence Prevention, Health Services and others ensuring that appropriate resources are available and postvention strategies are mobilized on behalf of the student and the campus community.
  • In conjunction with the Director of Case Management actively participates in Student Support Network (SSN) meetings.
  • Performs other duties as assigned.


Qualifications
  • Master’s Degree Required in Social Work, Counseling, or related field.
  • Demonstrated knowledge of crisis management, crisis intervention and case management skills.
  • Knowledge of health care, mental health care and services, crisis management prevention, and psycho educational and educational outreach and programming to students, faculty, administrators, staff and parents.
  • Demonstrated ability to initiate and coordinate services for students.
  • Demonstrated ability to interact effectively, productively and sensitively with students, faculty, administrators, staff and parents per the standards and practices related to the Family Educational Rights Privacy Act (FERPA) and other relevant policies and best practices.
  • Demonstrated ability to work effectively and collaboratively and provide leadership within a multidisciplinary organizational setting including on campus residential facilities, faculty, campus administration, campus health providers, off campus health care providers, law enforcement and the courts.
  • Demonstrated ability to review, understand and effectively apply complex policies and laws.
  • Outstanding written, digital, and oral communications skills.
  • Excellent interpersonal, cultural competency and conflict resolution skills.
  • Demonstrated ability to effectively collaborate with people of diverse backgrounds.
  • Ability to follow, implement and encourage others to follow Vassar College policies and state and federal laws.
  • Ability to encourage others toward common and individual goals and to motivate others to follow. Preferred Qualifications:


Direct experience working on college campuses with college-age clients. Two years experience post graduate degree providing direct case management services.
