Summary

The Section of Transition Medicine at Baylor College of Medicine is seeking an expert level, professional nurse who demonstrates a mastery level proficiency in delivering patient care in the ambulatory care setting. The Transition Medicine clinic sees adults age 19 and older with the following diagnosis: autism, spina bifida, cerebral palsy, and down syndrome. In this position, the individual will assist physicians with treatment room examination and procedures, schedule appointments, triage patient calls, complete prior authorizations, and refill requests. Serves as a resource for Department of Medicine, Registered Nurse I and II's and Medical Assistants.

Job Duties
  • Supervises Medical Assistants, rooms patients and takes vital signs, discharges patients, and manages clinic flow between clinic staff.
  • Performs intermittent/straight cathing for urine, suctioning of tracheostomy, gives injections and medications, cleans wounds, and completes all other necessary procedures as directed by the physicians.
  • Uses hoyer lift to weight patients who are non-ambulatory (hoyer lift training can be taught on the job).
  • Facilitates patient education and does Annual Wellness Visits with Medicare patients.
  • Completes prior authorizations for patients for various medications, supplies, and services.
  • Manages Methodist Urology prior authorizations for our patients receiving care there.
  • Handles all calls from patients, which includes triaging medical concerns, care coordination, patient education.
  • Responds to patient messages (via EMR).
  • Writes letters of medical necessity detailing the medical information of the patient to justify the need for supplies or services.
  • Manages desktop calls from patients.
  • Completes refill requests for patients.
  • Schedules patient appointments as directed by the physicians.
  • Precepts / orients new clinical staff.
  • Delivers staff education, clinical in-services and/or participates in clinical staff meetings based on the needs of the department.
  • Respects and supports patient rights and promotes positive family and patient relations.
  • Prioritizes and organizes work so that assignments are completed in an efficient and timely manner.
  • Maintains a professional and cooperative relationship among health care teams by communicating information, responding to requests, and building rapport by participating in team problem-solving methods.
  • Serves as a subject matter expert and clinical champion for areas of expertise within your specialty.
  • Conducts performance improvement activities that support the clinic / department's goals for quality, patient experience, patient safety, and/or efficiency.
  • Completes other task as assigned by the Practice Manager or Lead Nurse.


Minimum Qualifications
  • Associate's Degree or Bachelor's Degree in Nursing.
  • Two years of relevant experience as a licensed Registered Nurse (RN). Degrees in lieu of experience will not be accepted.
  • Current license as a Registered Nurse (RN) by the State of Texas Board of Nursing. Basic Life Support (BLS) Certification.
