Position Summary
The School of Nursing is seeking a qualified candidate to serve as a Project Manager on a federally funded award. The study seeks to:
• Promote care continuity
• Reduce low value utilization
• Reduce the burden of treatment for high-need, high-cost patients receiving care across health, specialty and social care sectors
About Us
The project will expand the evidence-based Care Transitions Intervention ( CTI ) to bring social sector providers as equal partners in HNHC care, refine CTI HIE capacity to include cross-sector care alerts and a personalized care plan, and evaluate the impact of the intervention from a patient and provider perspective.
