The Department of Medicine, Division of Endocrinology is seeking a Nurse Practitionerto work full-time in the clinical care and education of patients with diabetes at Johns Hopkins Hospital. Will work on the Inpatient Diabetes Management Service (IDMSand work collaboratively with resident physicians, fellows, staff nurses, and other nurse practitioners to provide consultative care for glycemic management of patients on both medical and surgical services.

The IDMS is operated under the management of an attending physician who is a member of the Division of Endocrinology, Diabetes and Metabolism in the Department of Medicine; the JHH-IDMS is a national benchmark for inpatient diabetes programs. In In addition to providing daily consultative service to patients in the hospital, the Nurse Practitioner will participate in initiatives of the hospital’s Glucose Steering Committee which is responsible for protocol and policy development and patient safety and quality systems interventions as they relate to glycemic management. A major focus of the work of the IDMS and GSC is patient and provider education. In particular, the nurse practitioner will, along with IDMS NP colleagues, assist with all aspects of the Diabetes Nurse Super user program to promote and support safe diabetes management for hospitalized patients.

The Nurse Practitioner will practice within the guidelines of the unit department and institution and in accordance with the Nurse Practitioner-Physician Written Agreement required and approved by the state of Maryland. The Nurse Practitioner will be granted staff privileges by the Johns Hopkins Hospital Medical Board; these privileges list specific tasks and are reviewed annually. The Nurse Practitioner will participate in training and evaluation led by the Division of Endocrinology, Diabetes, and Metabolism, and meet internal training guidelines prior to treating patients. The Nurse Practitioner will also be credentialed at Johns Hopkins Bayview Medical Center in the event cross-coverage of other Nurse Practitioners needs to be scheduled, or the needs of the service shift.

Ability to incorporate into practice theoretical concepts and advanced clinical skills with patients, families, and interdisciplinary health care team members. Analytical abilities are necessary to diagnose patients and to develop and implement plans of action. Advanced interpersonal skills are necessary to communicate effectively with all members of the interdisciplinary health care team to enhance collaboration and problem-solving. Excellent communication skills, both verbal and written, are necessary to provide information to patients and health care personnel. The Nurse Practitioner must be able to function effectively in a fluid, dynamic, and rapidly changing environment.

Essential Job Functions:

  • Perform 3-4 new consultations daily for hospitalized adults on medical and surgical services with diabetes/acute hyperglycemia.
  • Make recommendations for the initial glucose management plan.
  • See follow-up consults to make adjustments in the initial glucose management plan based on the patient’s evolving clinical condition
  • Manage inpatients who have insulin pumps as their primary mode of insulin delivery.
  • Document the diabetes management plan in the electronic medical record on a daily basis and communicating directly with primary teams.
  • Ensure appropriate plans have been made for patients at the time of discharge including: appropriate clinic follow-up appointments have been made and adequate diabetes treatment and testing supplies have been ordered.
  • Assist in the cross-coverage of patients followed by other members of the Inpatient Diabetes Management Service (i.e. vacation, illness).
  • Assist in the work of the Glucose Steering Committee.
  • Assist with nursing staff education regarding inpatient glucose management, by performing key roles in the Nurse Diabetes Super user program which is run by the IDMS nurse practitioner team.
  • Work collaboratively with the inpatient endocrine attending and fellow each month and with outpatient providers in the Diabetes Center.
  • Follow up with the patient’s primary care physician or established endocrinologist as need to ensure continuity of care.
  • Educate patients and families regarding the nature of their clinical problems, prognoses, and options for continued care.

Minimum Qualifications:

Master's degree in nursing; Registered Nurse license and Certified as a Nurse Practitioner. Must be licensed as NP in State of Maryland or other state where practicing. Additional experience may substitute for master's degree. Current CPR/ACLS certification required. Must maintain current licensure and certification during duration of employment. Completion of the JHH credentialing process is required prior to start date.

JHU Equivalency Formula: 30 undergraduate degree credits (semester hours) or 18 graduate degree credits may substitute for one year of experience. Additional related experience may substitute for required education on the same basis.

Preferred Qualifications:

  • Experience in diabetes management.
  • Excellent organizational skills and the ability to handle multiple tasks simultaneously.
  • Analytical ability necessary to evaluate health status of patient and to implement proper care plan.
  • Interpersonal skills necessary to communicate with other services.
  • Ability to work well as a member of a health care team with a high degree of self-motivation.
  • General computer knowledge and ability to use computerized medical information systems.
Physical Requirements:

Sitting in a normal seated position for extended periods of time. Reaching by extending hand(s) or arm(s) in any direction. Finger dexterity required to manipulate objects with fingers rather than with whole hand(s) or arm(s), for example, using a keyboard. Ability to lift patients, 50 lbs. or more. Ability to move standard equipment through a hospital or clinical environment.
