POSITION SUMMARY

The CARE Coordinator provides goal-oriented and strengths-based assessment, intervention, and coordination of services to students experiencing academic, personal, or medical difficulties in order to assist them in removing barriers to success and increasing their holistic well-being. In this role, the CARE Coordinator is a key resource in making referrals to, participating in, and delivering interventions from campus CARE Team and the Counseling Center.

PRINICIPLE ACCOUNTABILITIES

  1. Case Management Responsibilities:
  • Provide outreach to students referred by CARE Team or the Counseling Center to schedule appointments.
  • Evaluate students for holistic assessment of needs.
  • Provide follow-up appointments as appropriate to coordinate action plan, referrals, and support.
  • Connect students with the appropriate resources on and off campus.
  • Communicate and collaborate with appropriate staff and resources.
  • Document case notes in Titanium.

  1. CARE Team Responsibilities:
    • Attend and participate in CARE Team meetings by having case notes prepared on current students, reviewing new referrals, and contributing to the discussion.
    • Makes referrals to the CARE Team as appropriate for students working directly with case management.
    • Supports the CARE Team by providing interventions to students.

  2. Outreach and Awareness Responsibilities:
    • Creates marketing, awareness, and education campaigns to promote case management services.
    • Create and deliver educational presentations that are tailored to meet the needs of the audience.
    • Supervise and educate peer students to ensure they receive support and that their presentations are evidence based.
    • Coordinate with various campus offices to ensure coherent integration of messaging and duties.

  3. Departmental Responsibilities
    • Complete special projects or other duties as assigned by supervisor.
    • Assist in compiling end-of- semester and end-of-year reports.
    • Assist in developing and implementing strategic planning.
    • Participate in assessment efforts related to case management effectiveness.
    • Participate in other committees as assigned.


REQUIRED KNOWLEDGE, SKILLS AND ABILITIES
  • Demonstrated knowledge, sensitivity and ability to work with students from diverse ethnic, cultural and socio-economic backgrounds including LGBT students and students with disabilities.
  • Experience with organizational development and outreach.
  • Ability to solve problems effectively and creatively.
  • Demonstrated autonomy, effectiveness and sensitivity.
  • Excellent interpersonal and communication skills and the ability to work as a member of a collaborative, multidisciplinary team.


QUALIFICATIONS
  • Bachelor's degree


PREFERRED QUALIFICATIONS
  • Prior experience with providing interventions and creating treatment plans for clients
  • Specialty skills/certification related to mental health and wellness that could be provided to the student community in a group format
  • HECMA membership
  • Multicultural competence and interest/expertise in outreach and preventative programming
