The School of Public Health, Department of Health, Behavior and Society is currently seeking a motivated and experiencedCase Manager to provide case management and care coordination services at SPARC, a harm-reduction focused drop-in center and outreach program for women and non-men in Baltimore MD. SPARC service provision has pivoted during times of COVID-19 and is largely centered on providing mobile services through South and Southwest Baltimore. This mobile service provision includes pop-up outreach and medical services; the delivery of harm reduction, hygiene and food supplies; and a mobile HIV/STI testing and Hepatitis C clinic.

In addition to conducting HIV/STI prevention, harm reduction education and distributing supplies, the outreach team provides resource lists and micro-counseling to further support the complex needs of our service users, many of whom experience comorbidities and chronic homelessness. Outreach activities take place in Pigtown, Carrol Park, Mount Clare, and Brooklyn of South Baltimore.

SPARC is also an authorized Overdose Response Program (ORP) and a Syringe Service Program (SSP), to best meet the needs of service users who use opioids. All staff are trained overdose response and reversal as well as drug user health, safer injection practices and trauma informed care.

Specific Duties and Responsibilities:

  • Provide in-person and virtual support and follow-up to connect clients to available resources.
  • Assist guests in determining need, and eligibility for, internal and external therapeutic, medical, psychiatric, and social services.
  • Connect guests directly to service providers, including scheduling appointments, arranging for transportation to and from services, and sending follow-up reminders.
  • Research insurance information relevant to the referral process on behalf of guests. Assist guests with insurance enrollment, if needed.
  • Advocate for guests with medical and social service providers to reduce barriers and enhance access to care.
  • Follow up with referred guests, as needed, to ensure continuity of services and to evaluate the quality of care provided.
  • Maintain records of resource referrals and on-going client care.
  • Communicate regularly with senior staff to provide progress reports, updates, and feedback.
  • Distribute promotional items, Naloxone, and safer injection equipment, while also providing relevant risk reduction education and counseling in a non-judgmental way.
  • Engage in harm reduction-based street outreach throughout Baltimore City during the day and at night.
  • Develop and facilitate support groups, topic-based workshops, and skill-based educational groups
  • Build rapport and relationships with community members, including sex workers, community associations, and local business owners.
  • Drive the outreach vehicle and act as a team leader on outreach shifts.
  • Work in the Drop-In Center, assisting with client intake, laundry and shower services, clinical services, as well as other SPARC services and activities.
  • Perform other tasks and duties as assigned.

Minimum Qualifications:

Bachelor's degree in social work or related discipline. One year of related human service experience, preferably in case management.

Preferred Qualifications: Prior field work experience preferred. Experience conducting outreach with diverse populations including sex workers and drug users strongly preferred. Successful candidates will undergo training sessions prior to working in the community.

Physical Requirements:

  • Able to lift and carry up to 20lbs
  • Able to comfortably step into and out of a large transit van.

The pre-employment physical for positions in clinical areas, laboratories, working with research subjects, or involving community contact requires documentation of immune status against Rubella (German measles), Rubeola (Measles), Mumps, Varicella (chickenpox), Hepatitis B and documentation of having received the Tdap (Tetanus, diphtheria, pertussis) vaccination. This may include documentation of having two (2) MMR vaccines; two (2) Varicella vaccines; or antibody status to these diseases from laboratory testing. Blood tests for immunities to these diseases are ordinarily included in the pre-employment physical exam except for those employees who provide results of blood tests or immunization documentation from their own health care providers. Any vaccinations required for these diseases will be given at no cost in our Occupational Health office.
