General Summary: Coordinates clinical activities for and provides nursing case management services, and provides primary care nursing to assigned patients.

Specific Duties and Responsibilities:

  • Coordinates the clinical activities for and provides nursing case management services to patients who are seen in The John G. Bartlett Specialty Practice (Infectious Diseases Clinic) from the initial visit until termination of services.
  • Counsels, supports, educates, and coordinates resources needed by patients and their family members.
  • Provides primary care nursing to assigned patient panel of clinic patients, and provides backup coverage to other nurses. Primary nursing begins with the first encounter with the patient and continues throughout the course of care.
  • Independently reviews laboratory results for abnormalities; communicates abnormalities to medical provider and implements intervention plans when clinically warranted in collaboration with provider.
  • Facilitates and schedules urgent appointments for patient testing and specialty care.
  • Provides both telephonic and in person clinical triage during clinic hours. Clinically assesses need for patient diagnostic testing prior to evaluation by medical clinician. Responds to calls and drop-in visits from patients requesting urgent appointments and uses nursing judgment to assess acuity. Schedules urgent appointments in clinic or refers patients to the Emergency Department, as indicated by acuity.
  • Responds to and provides clinical care during patient emergencies in the clinic.
  • Triages calls from the Emergency Department, Infectious Disease Fellows, and other health care providers. Determines and implements follow-up plans for patients requiring medical treatment.
  • Arranges for patient hospitalizations by coordinating the interaction between the attending physician, central bed management, Infectious Diseases Fellow, and inpatient unit. May provide direct clinical care to patients awaiting admission in the Treatment Room.
  • Assigns work to medical assistants, as directed by the Nurse Manager.
  • Provision of Treatment Room services as needed.
  • Educate patients about HIV disease, viral hepatitis, general infectious diseases and other comorbid conditions, disease state management and prevention, medication adherence and effective self-care.
  • Monitors and collaborates with external agencies and community based organizations to maintain continuity of care and prudent use of resources.
  • Oversees case management activities of home care nurses and facilitates communication between family, home care nursing, physician, and patient. Liaises with clinic providers to determine orders for home care nursing of patients in the community. Communicates home care orders to appropriate home care agencies, and coordinates care.
  • Maintains current knowledge of all JHH, JHU, and Bartlett Specialty Practice policies and procedures related to unit operation and assumes responsibility for appropriate application and compliance.
  • Participates in quality and safety initiatives for the Department and Clinic.
  • Assumes responsibility of clinic-based projects as designated by Nurse Manager.
  • Engages in professional development activities to grow/strengthen specialization in HIV nursing.
  • Attends regularly scheduled meetings and HIV-related educational conferences as requested by Nurse Manager.
  • Successfully manages fast-paced clinical environment with frequent interruption in planned work. Regularly interfaces with patients and a multidisciplinary care team.
  • Documents care coordination and patient care activities in the electronic medical record. Effectively utilizes electronic medical record and electronic medication prescription programs.
  • Adapts well to advancing, evolving work environment.
  • Participates in educational training sessions with individual patients and groups.
  • Develop tailored care plans for patients with the input of prescribers and other adherence staff to address medication adherence issues.

Education:

  • Baccalaureate degree in Nursing from an accredited School of Nursing preferred. Applicants with an Associate Degree in Nursing will be considered.

Required Licensure, Certification:

  • Registered Nurse (RN) Required. Maryland State or Compact License Required.
  • BLS Certification provided by AHA, American Red Cross, or Military Training Network.
  • Completion of the JHH credentialing process is required prior to start date.
  • All credentials must be maintained with active status.

Experience:

  • One year of experience as a licensed Registered Nurse preferred.
  • Experience in a healthcare or public welfare setting with knowledge of HIV prevention, HIV treatment, substance abuse, and/or psychiatric illness preferred.

Special Knowledge, Skills, and Abilities:

  • Must possess knowledge of nursing process; social; behavioral, biological, and physical science. Knowledge of current health care environment.
  • Must be able to operate a hospital based electronic patient care record.
  • In-depth knowledge of the clinical manifestations and management of HIV infection.
  • Adept at the clinical assessment of patients living with and at risk for HIV infection.
  • Ability to take independent action to care for ambulatory patients who are acutely ill awaiting provider evaluation.
  • Strong interpersonal skills required to interact with health care providers and staff from many disciplines.
  • Knowledge of lesbian/gay/gender identity issues, and resources.
  • Able to work with culturally diverse patient, staff and community populations.
  • Strong organizational skills with attention to detail and follow through.

This may include documentation of having two (2) MMR vaccines; two (2) Varicella vaccines; or antibody status to these diseases from laboratory testing. Blood tests for immunities to these diseases are ordinarily included in the pre-employment physical exam except for those employees who provide results of blood tests or immunization documentation from their own health care providers. Any vaccinations required for these diseases will be given at no cost in our Occupational Health office.
