GENNERAL SUMMARY:
To provide administrative leadership and clinical direction for the social work/case management (sw/cm) program of the Johns Hopkins University Adult HIV/AIDS Services (JHUAS). Oversight includes ambulatory (Bartlett Specialty Practice) and satellite practice sites. Responsibility to develop, implement and maintain sw/cm services to meet the needs of the patients and staff. Serve as Manager of Ryan White contracts in the JHUAS and assist the Director of Ryan White services in the implementation and monitoring of Ryan White funded services provided by JHUAS.


1. Bartlett Specialty Practice:The Bartlett Specialty Practice provides medical and sw/cm services to approximately 3,000 medically and socially complex adults with HIV/AIDS and Hepatitis. Sw/cm services, performed by 9 case managers and 3 social workers, include psychosocial assessments, referrals to substance abuse and mental health treatment programs, assisting clients obtain and maintain health insurance within the structure imposed by state and federal guidelines, as well as other direct services.
2. Satellite Clinics:The Bartlett Specialty Practice has satellite sites at the Greenspring Station site of Johns Hopkins Medicine and at the Johns Hopkins Howard County Medical Pavilion. These sites serve approximately 1,000 adult patients. One case manager provides the same comprehensive sw/cm services offered at the Bartlett Specialty Practice to patients at the satellite sites.
3. Ryan White Care Act:The Ryan White Care Act is a federal grant awarded to the Bartlett Specialty Practice that provides primary medical and support services to people with HIV/AIDS. The clinic has received the funding since 1996 to offer medical care, case management, transportation, medication, and other wraparound services.


This individual will perform all hiring and staff oversight at the Bartlett Specialty Practice and the satellite sites. Additionally, this individual will supervise the Senior Clinical Social Worker of the Bartlett Specialty Practice who is responsible for the day-to-day operations of the sw/cm team at the Bartlett Specialty Practice.
In addition to personnel-related responsibilities, this individual will administer and oversee budgets for the Ryan White funding received by the clinic (approx. $4 million). This includes monthly and quarterly reporting to Baltimore City Health Department and the Department of Mental Health and Hygiene for the state of Maryland. The individual will assist the Director of Ryan White services in grant writing for annual renewal of this funding source.


ESSENTIAL JOB FUNCTIONS
Administrative:
Develop and implement programs to provide needed sw/cm services for Bartlett Specialty Practice and satellite clinic patients and to secure funding for these services. Programs include:

  1. Insurance coverage under the Affordable Care Act: Develop and maintain program to ensure that clinic patients without health insurance obtain insurance under the Affordable Care Act: identify eligible patients, train sw/cm staff, provide outreach and education to patients, enlist patient navigator to enroll patients into available insurance programs
  2. Billing for Social Work Services: Implement billing for social work services (psychosocial assessments, mental health screenings, substance abuse assessments, treatment adherence educations, safe sex awareness, and crisis intervention) Train staff on appropriate patient documentation in EPIC, ICD codes, and the different services that qualify for billing.
  3. Enhancing reporting for ongoing grants: A critical component of maintaining grants for clinical services is to provide city, state, and federal agencies with reports on how funding and services are being allocated. Transition data from multiple information sources (MS Access database, EPIC, and EPR) to one data source (EPIC) to generate reports.

Additional administrative responsibilities include:
1. Implement effective policies and procedures to guide clinical and administrative practice
2. Develop patient care programs provided by all sw/cm
3. Monitor patient care programs to ensure appropriate delivery of service
4. Hire, train/educate, supervise, and evaluate program staff
5. Collaborate and coordinate with Division administration to integrate services into patient care programs
6. Develop, monitor, and maintain information systems for appropriate data collection and review

Planning:
1. Ensure maintenance of an adequate and supportive work environment marked by good teamwork, mutual respect, individual initiatives, cultivation of new enterprise and ideas, and successful resolution of problems
2. Monitor Division needs in order to make appropriate changes in sw/cm program
3. Maintain acute awareness of changing health insurance and entitlement programs that may be accessible to patients

Clinical:
1. Ensure professional quality of the clinical program
2. Evaluate and measure clinical operations
3. Provide clinical consultation to professional JHMI community when appropriate
4. Provide clinical direction and leadership to sw/cm staff through consultation, supervision, and educational initiatives
5. Provide clinical supervision for social workers seeking advanced licensure
6. Develop innovative practice methods when appropriate which are responsive to the changing developmental and social needs of the patient population
7. Provide direct patient care, including coverage for sw/cm staff

Community Functions:
1. Establish relationships with social, political and professional communities relevant to sw/cm program
2. Act as a member of boards or committees of outside agencies within the community as they relate to the patient population
3. Initiate contact and collaborate with community programs to develop new programs and services
4. Maintain active membership on the JHH Social Work Council and participate in Hospital social work programs as indicated
5. Active member of the Greater Baltimore City Planning Council, Continuum of Care Subcommittee
6. Attend meetings in the community associated with providing Ryan White services

MINIMUM QUALIFICATIONS (MANDATORY):

Education:
MSW from accredited school

Licensure, Certification, Registration:
Maryland LCSW-C

Experience:
5 years (of which 3 years are mental health /health related) post MSW


Special Knowledge, Skills, and Abilities:
Knowledge of procedures, systems and organizational objectives used within social work and Hospital system; a high level of proficiency and skill in providing social work services in mental health/health care setting, and comprehensive knowledge of community resources

Analytical ability to assess patient needs at a multi-system level; to develop associated treatment modalities at a clinically complex level; and to provide effective counseling.

Analytical ability to develop and initiate clinical practice standards

A sound and thorough knowledge of psycho-dynamics and social theories with a broad application to social work practice

Analytical ability to identify organizational needs and obstacles and make recommendations that impact the delivery of optimal patient care and social work practice

Knowledge of the medical and psychosocial issues through the course of illness

Knowledge of current health care environment

Understanding of management practices and methods to include development of budgets; knowledge of human resource policies and practices

Knowledge of national, state, and local policies and how they impact on health care and social work practice

Knowledge of current trends, philosophies and theories related to social work education and practice

Expertise to serve as consultant throughout the Hospital regarding the impact of social context on the outcome of health care

Effective leadership

Effective communication, both written and oral

Ability to collect and analyze data

Consultant to other professionals concerning the environmental and psychosocial conditions of patients and families

Information management skills to communicate, explain, interpret, share and/or present information

Monitor clinical performance through case consultation, record review and regular supervisory sessions

There is possible exposure to communicable diseases, hazardous material, and pharmacological agents, with little likelihood of harm if established health precautions are followed. Deadlines and reporting requirements based on funding are frequent.
