Program Coordinator

General Summary/Purpose

The mission of the Johns Hopkins Brancati Center for the Advancement of Community Care Diabetes Prevention Program (DPP) is to reduce the incidence of type 2 diabetes in the community. The Brancati Center works cooperatively with various community-based partners to deliver both in-person, distance learning, and virtual Diabetes Prevention Programs. The Brancati Center provides clinical expertise and staff, health coaches, quality assurance measures, and program and clinical management for all DPP programs.

The DPP Program Coordinator will be responsible for managing the intervention components of the Diabetes Prevention Programs and serving as back-up lifestyle coach for the National Diabetes Prevention Program. Under the direction of the DPP Program Director, the Program Coordinator will participate in program development, implementation, operations oversight, data monitoring and reporting, and quality control management. Under the direction of the Program Manager, the Coordinator will be responsible for organizing community outreach events and managing existing community partnerships. The Coordinator will work closely with Brancati Center staff to ensure the DPPs achieve quality outcomes and with sustainable community programs. This position will directly interact with community members, Lifestyle Coaches, and program participants, as well as public health workers, physicians, and other payer communities and referral networks to promote awareness of the DPP and manage participant referrals. The Coordinator will report to the Brancati Center Program Manager, DPP Program Director, and the Director of Community Partnership.

Specific Duties & Responsibilities

Under the direction of the Program Manager and DPP Program Director, the Coordinator will oversee and manage all operations related to lifestyle groups, as well as special projects and initiatives relating to the DPP. The Coordinator will play a key role in developing and maintaining partnerships with various community groups and members. Other duties will include assisting the Program Director with the evaluation process and quality control management of the program and coaches while proposing and revising processes as necessary to achieve successful program outcomes. The position will provide support to the entire DPP team.

DPP Program Development

During program development the Coordinator will (in collaboration with the Brancati Center team):

  • Work with the Program Manager to manage community partnerships for implementing the DPP.
  • Establish relationships with various stakeholders, including individuals from public health, healthcare, payers, and other referral networks.
  • Serve as liaison, ambassador, and advocate for the lifestyle change program with members in the community.
  • Work with the Directors and staff to assist in the program development phase of all DPP in-person and distance learning groups.
  • Work with the Program Manager to adhere to and monitor program budgets.
  • Assist the Program Director in the drafting, submission and maintenance of IRB documents.
  • Work with community at off-site locations to coordinate screening and recruitment procedures.
  • Oversee all phases of DPP program recruitment, screening, and enrollment with a focus on recruiting eligible participants to achieve enrollment targets.
  • Conduct behavioral interviews as needed during participant recruitment for the DPP program.
  • Assist the Program Director with creating and implementing internal tracking mechanisms for the recruitment and enrollment phases.
  • Assist the Program Director with establishing and updating policies and procedures related to carrying out all phases of DPP implementation.
  • Become familiar and administer all duties in compliance with the National Diabetes Prevention Recognition Program Standards.
  • Be trained as a certified CDC Lifestyle Coach.
  • Become familiar with Medicare and Medicaid rulings for the DPP to ensure adherence to CMS policies.
  • Become trained in EPIC for participant registration, scheduling, and documentation.
  • Order DPP supplies and maintain appropriate inventory.
  • Prepare all DPP materials for DPP groups, working with other DPP members, including coach assistants, as they are available.
  • Organize program materials and delivery of the DPP program with adherence to a CDC-approved curriculum.
  • Assist the Program Director with the development of special curriculum materials.
  • Help establish the program structure and timeline for various stages of DPP implementation: group delivery, coach oversight, data monitoring and reporting.
  • Develop a master schedule of the in-person group classes offered by the organization and troubleshoot and solve issues with scheduling as they arise.
  • Work with the Data Manager to help create and implement internal tracking systems for evaluating and reporting program outcomes.
  • Plan and coordinate the deployment of concurrent DPP groups at multiple locations within program time restraints.
  • Learn the REDCap database and develop forms and reports for data outcomes.

Program Implementation

During program implementation, the DPP Coordinator will (in collaboration with the Brancati Center team):

  • Assist the Program Director with establishing the structure and initial framework for all new in-person DPP groups.
  • Consult with the Directors to review program goals, timelines and procedures for implementation
  • Serve as the back-up in-person or distance learning coach during the program duration: facilitate lifestyle sessions/focus groups for changing eating and exercise behaviors; utilize teaching tools specific to participant populations served; and facilitate make-up sessions as needed.
  • Collect and enter DPP participant data in accordance with CDC guidelines and Brancati Center policies.
  • Assist in double data entry and quality control of data as needed in collaboration with Data Manager.
  • Deliver and follow the NDPP curriculum and adjust to meet the needs of the participants.
  • Review program data for weight loss and attendance to coordinate case management meetings for lifestyle coaches.
  • Participate in behavioral case management meetings to evaluate participant behavior, identify problems, and implement corrective actions.
  • Demonstrate problem-solving skills as they relate to participant care activities and act as a role model for Lifestyle Coaches.
  • Provide support and guidance to the coaches and monitor the quality of support that they provide to the DPP participants.
  • Ensure adequate class coverage with staffing/group schedules and troubleshoot issues as they arise.
  • Monitor retention and commitment of program participants.
  • Review program data to ensure it meets quality performance standards.
  • Assist the Program Director with creating and implementing quality assurance measures to ensure coach compliance with CDC’s Diabetes Prevention Recognition standards.
  • Assist the Program Director with training events for new Lifestyle Coaches.
  • Ensure compliance with JHU operational policies, applicable laws and regulations and those governing privacy and data security (HIPPA regulations).

Minimum Qualifications(Required)

High school diploma/GED and five (5) years of progressively responsible related experience.

* JHU Equivalency Formula:30 undergraduate degree credits (semester hours) or 18 graduate degree credits may substitute for one year of experience. Additional related experience may substitute for the required education on the same basis. For jobs where equivalency is permitted, up to two years of non-related college course work may be applied towards the total minimum education/experience required for the respective job. *

Preferred Qualifications

Bachelor’s Degree in Nutrition, Health Science, Public Health, Behavioral Psychology, Kinesiology, or related degree preferred and may substitute for some experience, to the extent permitted by the JHU equivalency formula.

One year of program management experience, preferably in health care.

One year of experience in conducting behaviorally oriented diet/exercise groups or other educational program.

Clinical experience with prediabetes, diabetes, hypertension, and/or obesity; familiarity with motivational interviewing and knowledge of the principles of behavior change, and cultural competency regarding the needs of minority populations.

Excellent computer skills: Microsoft Office suite, Zoom
