The Department of Oncology, Thoracic MDC is seeking aClinical Nurseto collaborate with a multidisciplinary health care team to provide direct patient care based on the nursing process and optimize team performance.

The Multi-Disciplinary Clinical Coordinator for care, outreach and education of the thoracic multidisciplinary cancer program will be responsible for enhancing patient and provider satisfaction, improving access to services, and assuring that patient data is captured and shared among the team members. This person will be charged with providing materials and programming to increase awareness and education about this particular tumor type.

This individual will serve as an integral member of the multidisciplinary team, working closely with the program champion, and a wide ranging group of specialists and providers. Further, the coordinator will be responsible for maintaining or developing a way to collect, analyze and present data related to the size and scope of the program, and will be knowledgeable about the particular tumor, cancer in general, the program and Johns Hopkins research related to this tumor. This position will be expected to be a part of the survivorship efforts in thoracic cancer.

Summary of Responsibilities:

  • Coordinating care for assigned patients on a shift to promote the achievement of clinical outcomes.
  • Promote improvements in unit processes and cost effective use of resources.
  • Accountable for own decisions regarding patient care.
  • Analyzes patient care data to deliver and evaluate care.
  • Identifies impending changes in patient condition and individual performance issues

Primary Duties and Responsibilities:

  • Receive referrals from diagnostic clinic and our attending physicians.
  • Speak with patients/families to explain MDC process.
  • Coordinate additional diagnostic testing needed for patients prior to being seen.
  • Coordinate scheduling appointments with NPRO.
  • Create weekly summary sheet assigning fellows/residents ideally with those that will be providing future care.
  • Mass email the assignment/summary sheet to group afternoon before MDC.
  • Coordinate MDC the day of:
  • Teach class to all patients/families
  • Coordinate attending visits to various rooms (keep track of who needs to see who)
  • Pend Orders/schedule additional testing
  • Wrap-up discussion/clarify plan with each patient/family and review AVS
  • Discuss/relay new test results with appropriate team members and patients.
  • Adjust plan accordingly if needed and explain to patient or confirm who will tell patient.
  • Assist with transition of care if outside of JHH (fax reports to outside oncologists).
  • Assist with transition of care from induction therapy to surgery when appropriate.
  • Maintain schedule of attending and mid-level providers.
  • Send reminders emails to attending every week to complete notes.
  • Serve as clinical resource for all disciplines particularly the nursing and support staff.
  • Serve as link to research nurses and coordinators for clinical trials.
  • Serve as community resource for educational events.
  • Serve as resource with marketing and Development office for fundraising efforts.
  • Maintain Patient Guide educational folders.
  • Minimum Qualifications:

    Individual must be a registered nurse, licensed in the State of Maryland or state where practicing. Bachelor's degree in nursing or related discipline preferred. Current American Heart Association or American Red Cross CPR certification required. Must maintain current licensure and certification during duration of employment. Acceptable completion of the JHH credentialing process is required within two weeks of start date.

    Additional Knowledge, Skills, or Abilities / Competencies:

    • Knowledge of nursing process, social, behavioral, biological and physical science.
    • Knowledge of current health care environment.
    • Ability to analyze data to synthesize and summarize information and make judgments regarding patient care.
    • Highly effective verbal communication and interpersonal skills to establish working relationships that foster optimal quality patient care.
    • Ability to document legibly using a writing instrument.
    • Ability to use a computer keyboard to input and obtain data.
    • CPR and AED certified.
    • Ability to properly conduct general physiologic monitoring and patient care equipment such as an EKG.
    • General physiologic monitoring and patient care equipment such as defibrillator and glucometer monitor.

    Physical Requirements:

    Sitting, standing and walking for extended period. Reaching by extending hand(s) or arm(s) in any direction. Finger dexterity required to manipulate objects with fingers rather than with whole hand(s) or arm(s). Ability to move standard equipment through a hospital or clinical environment.

    Work Conditions:

    • Possible exposure to communicable diseases, hazardous materials, radiation and pharmacological agents.
    • Work requires standing and/or walking for extended periods of time.
    • Work requires lifting and/or positioning patients and transporting wheelchairs, beds and related equipment.
    • Work requires manual dexterity and hand-eye coordination to perform patient care procedures.
    • Work may require contact with aggressive and/or combative patients.

    Employees in this position will be required to adhere to work availability and scheduling requirements. This may include, but is not limited to availability status, mandatory overtime, on call and shift work.
