The Department of Medicine, Division of Infectious Diseases is seeking aNurse Practitioner Manager. The Medical Director oversees clinical services within the HIV primary care program (Early Intervention Initiative), the PrEP program serving HIV-seropositive and at-risk patients, and the hepatitis C screening and treatment program (CHARM) at Baltimore City Health Department Sexual Health and Wellness Clinics. Also oversees the activities of the HIV Directly Observed Therapy (DOT) Program which provides home-based treatment adherence services to HIV-seropositive patients referred from Baltimore City HIV clinics.

These professional services are provided through a contract with the Johns Hopkins University School of Medicine. Duties include direct patient care as well as on-site supervision of advanced practice clinicians to ensure that standards for excellence can be met in HIV, PrEP, hepatitis C, STD, and buprenorphine treatment services.

The Medical Director will work collaboratively with members of the multidisciplinary care team to provide care throughout the spectrum of HIV disease and hepatitis C. Additionally, the Medical Director will collaborate with the HIV/STD Prevention team at the BCHD as well as other Baltimore City partners to advance innovative clinical services related to HIV and hepatitis treatment and prevention. The Medical Director reports directly to the Clinical Chief, and works in collaboration with the STD Medical Director, Mobile Clinical Services Medical Director, Clinical Chief(s), and Assistant Commissioner to ensure services at the Sexual Health and Wellness Clinics are high-quality and integrated across disciplines.

Managerial Duties and Responsibilities:

  • Directly supervises the EII and PrEP/DOT program managers, who supervise patient advocates, social workers, peer navigators, nurses, and community health workers totally approximately 18 staff. Additionally, directly supervises nurse practitioners and physician assistants who provide primarily HIV, hepatitis C, and PrEP care, totally approximately 3 clinicians.
  • Oversees management of Ryan White grants and other relevant grants for continued funding of EII, PrEP, and HIV DOT programs. This includes grant preparation, budget development and management, required deliverables and meetings, and grant reporting.
  • Participates in leadership meetings with Assistant Commissioner of Clinical Services, STD Medical Director, Mobile Clinical Services Medical Director and Clinical Chiefs.
  • Supervises the clinical care of the EII, CHARM, and PrEP programs delivered by nurse practitioners and physician assistants to ensure care is delivered in accordance with clinical guidelines.
  • Provides direct outpatient primary care to an active panel of at least 30 HIV+ patients, as well as hepatitis C treatment, PrEP, and buprenorphine management as appropriate.
  • Provides medical consultation on medically complex cases to nurse practitioners and physician assistants as needed.
  • Trains newly hired, nurses, nurse practitioners and physician assistants in clinical HIV and hepatitis C care and prevention.
  • Performs chart reviews and periodic evaluations of clinician-patient encounters to monitor quality of HIV, hepatitis C, and PrEP services delivered.
  • Oversees the delivery of mental health care by on-site psychotherapist(s) and counselor staff to ensure that it meets clinical practice standards and that psychiatric consultation is sought when necessary.
  • Participates in the development and revision of clinical practice guidelines and performance improvement plans for the STD clinical program as they relate to HIV clinical and preventative services.
  • Participates in multidisciplinary case management meetings focusing on complex HIV patients to ensure that decisions related to support services are medically appropriate.
  • Schedules and/or directly provides in-service trainings for staff in topics related to HIV and hepatitis C management and adult primary care, and participates as a guest lecturer in the Prevention Training Center STD intensive course.
  • Oversees all HIV DOT activities, including supervision of DOT program manager/case manager, review of DOT referrals, and medical consultation with team and referring providers.
  • Use operational data to guide decisions related to HIV treatment and prevention services. Develop and implement clinical protocols. Support the STD medical director and clinic manager in day to day clinic operation
  • Works collaboratively with a multidisciplinary team at the BCHD to develop and implement city-wide efforts related to the HIV and hepatitis C care continuum. Collaborate with the Baltimore City Health Department STD/HIV Prevention teams including linkage to care, DIS, outreach, social innovations, PrEP/IMPACT, and surveillance. Collaborate with Baltimore City partners to further access to quality HIV and hepatitis C treatment and prevention clinical services.
  • Periodically prepare abstracts for presentation at appropriate conferences and prepare manuscripts for publication in peer-reviewed journals.
  • Moderates patient complaints with staff or services offered in a mindful and empathetic manner.
  • Work with learners from various clinical institutions in Baltimore for clinical and research rotations.
  • Maintain flexibility to expand scope of work to include tuberculosis management as needed.
  • Other duties as required.

Clinical Duties and Responsibilities

  • Examines and evaluates all new and return patients and documents findings. This is to include as needed: medical history, physical examination, written orders for medication and diagnostic tests, and review of results of bacteriologic and laboratory results.
  • Implements protocols for treatment, monitoring adverse effects, and emergency procedures, recognizing that the condition of individual patients may occasionally require a medical decision to alter certain aspects of these protocols.
  • Completes all forms requiring the attention of a clinician, i.e. insurance, vocational, rehabilitation, social services, automated information management input documents and abstracts patient records as required.
  • Provides consultations, makes patient referrals and arranges general hospitalizations with appropriated medical care providers.
  • Participates in in-service training and supervises clinical training of approved students and physicians in training.
  • Participates in research activities authorized by JHU and BCHD.
  • Counsels patients, providing disease information about treatment, prevention, follow-up care, sex partner referral and contraception as appropriate.
  • Provides emergency services to the Department as needed.
  • Maintains a current and valid license in accordance with the respective governing board.

This position description provides a summary of the major duties and responsibilities by the individual in this position. The incumbent may be asked to perform other clinical or administrative tasks not specifically written in this position, but considered important to the success of the BCHD Clinics or to the Johns Hopkins mission of excellence in teaching, research and patient care. These extra duties may be assigned or deemed necessary by the Chief of Clinical Services, the Assistant Commissioner of the Bureau of Clinical Services, the Commissioner of Health, or the Director of the JHU Division of Infectious Diseases.

Additional Knowledge, Skills, and Abilities:

  • Knowledge of STD/HIV/HCV/TB disease, treatment, and resources
  • Maintain certification as an HIV Specialist by the American Academy of HIV Medicine or board certification in Infectious Diseases.
  • Waiver to prescribe buprenorphine.
  • Ability to work with culturally diverse patient, staff and community populations.
  • Ability to work in multidisciplinary, multi-organization team settings.
  • Strong organizational skills with attention to detail and follow through.
  • Experience with grant and budget management.
  • Strong analytic skills and ability to analyze and interpret data to present in a scientific setting.
  • Knowledge of Spoken Spanish is preferred.

Supervises:

NPs, Nurse Manager, 10-11 BCHD social workers, Patient Advocates, and Community Health Workers.

Minimum Qualifications:

Master's degree in nursing; Registered Nurse license and Certified as a Nurse Practitioner. Must be licensed as NP in State of Maryland or other state where practicing. Require minimum of 3 years experience in related clinical area, and obtained proficiency in area of specialty. Prefer at least 5 years related experience. Supervisory experience preferred. Residency or fellowship programs in area of specialization may substitute for some experience. Current CPR certification required. Must maintain current licensure and certification during duration of employment. Completion of the JHH credentialing process is required prior to start date.

JHU Equivalency Formula: 30 undergraduate degree credits (semester hours) or 18 graduate degree credits may substitute for one year of experience. Residency or fellowship programs in area of specialization may substitute for some experience..

Physical Requirements:

Sitting, standing and walking for extended period. Reaching by extending hand(s) or arm(s) in any direction. Finger dexterity required to manipulate objects with fingers rather than with whole hand(s) or arm(s). Ability to move standard equipment through a hospital or clinical environment.
